尿膀胱癌抗原作为膀胱肿瘤标志物的临床评价

摘　　要：为评价尿膀胱癌抗原(urinary bladder cancer antigen,UBC)在膀胱移行细胞癌(BTCC)中的诊断价值,采用ELISA法对53例BTCC患者、25例泌尿系统良性疾病患者和13例健康志愿者进行UBC检测,并同时行尿细胞学检查。结果显示:(1)BTCC患者UBC平均含量为26.26±28.49μg/L,与泌尿系统良性疾病患者和健康志愿者相比(9.41±9.63μg/L、1.73±0.79μg/L),均有显著性差异(P均<0.01)。(2)以7.5μg/L为最适临界值时,UBC诊断BTCC的敏感性和特异性分别为86.8%、76.3%,与尿细胞学检查(32.1%、97.4%)相比,均有显著性差异(P均<0.01)。结论:UBC具有简便、敏感和无创的特点,可作为辅助诊断BTCC的尿肿瘤标志物。To determine the diagnostic efficacy of urinary bladder cancer antigen （UBC） as the urinary tumor marker for transitional cell carcinoma of the bladder （BTCC）, an enzyme-linked immunosorbent assay （ELISA） was performed to measure the concentration of UBC in urine from 53 cases of BTCC, 25 cases of urological benign disease and 13 cases of healthy individuals, and cytology was carried out for each patient. The results showed that the UBC values in the BTCC patients （26.26±28.49μg/L） were significantly higher than those in the urological benign disease patients （9.41±9.63μg/L） and the healthy individuals （1.73±0.79μg/L）. At a threshold value of 7.5μg/L, the sensitivity of UBC （86.8%） for detection of BTCC was significantly better than that of the urinary cytology （32.1%）, but the specificity of urinary cytology （97.4%） was better than that of UBC （76.3%）. The study indicates that UBC is a simple, sensitive and noninvasive urinary tumor marker for BTCC.
