摘 要:背景:肠易激综合征(IBS)的发病机制不明,炎性细胞因子表达失调可能在其中发挥一定作用。目的:研究IBS患者血浆炎性细胞因子白细胞介素(IL)-6、IL-18、IL-13的表达。方法:选取42例IBS患者和20名对照者,以酶联免疫吸附测定(ELISA)检测血浆IL-6、IL-18、IL-13水平。结果:与对照组相比,腹泻型IBS组血浆IL-6和IL-18水平、便秘型IBS组血浆IL-18水平显著升高(P均<0.05)。腹泻型和便秘型IBS组血浆IL-13水平与对照组相比无显著差异。感染后IBS(pIBS)组血浆IL-6、IL-18水平较对照组显著升高(P<0.05),pIBS组和非pIBS组血浆IL-13水平与对照组相比无显著差异。结论:IBS患者血浆促炎细胞因子水平升高,感染因素影响了腹泻型IBS血浆细胞因子的变化,促炎细胞因子可作为IBS的潜在生物学标记物。Background: The pathogenesis of irritable bowel syndrome (IBS) is not completely clarified. The imbalance of inflammatory cytokines may play some roles. Aims: To study the expressions of peripheral blood inflammatory cytokines interleukin (IL)-6, IL-18, IL-13 in patients with IBS. Methods: Forty-two patients with IBS and 20 controls were enrolled. The expressions of plasma IL-6, IL-18, IL-13 were determined by enzyme-linked immunosorbent assay (ELISA). Results: Compared with control group, expressions of plasma IL-6, IL-18 in diarrhea-predominant IBS (D-IBS) group and IL-18 in constipation-predominant IBS (C-IBS) group increased significantly (P all 〈0.05). No significant differences were found in plasma IL-13 level between the two IBS groups and control group. Compared with control group, plasma IL-6, IL-18 levels in postinfeetious IBS (pIBS) group were significantly higher (P〈0.05). No significant differences were found in expression of plasma IL-13 between pIBS group, non-pIBS group and control group. Conclusions: Levels of plasma proinflammatory cytokines increase in IBS patients. Infection factors can affect the change of plasma cytokines in D-IBS, and proinflammatory cytokines may be used as potential biomarkers for IBS.
