康复治疗对脑卒中偏瘫患者平衡功能和日常生活能力的影响

摘　　要：目的：探讨系统康复治疗对早期及恢复期脑卒中偏瘫患者平衡功能和ADL的影响。方法：60例发病2-12周的脑卒中偏瘫患者分为A组31例和B组29例,均在常规内科治疗的基础上,A组给予系统的康复训练,包括运动疗法、肌电生物反馈及动静态平衡仪训练;B组以被动训练为主,辅以器具的应用。2组分别于入组时、治疗4、8周末采用Berg平衡量表（BBS）、功能独立性评定表（FIM）进行评定。结果：治疗4周后BBS及FIM评分与治疗前比较2组均有提高（P〈0.05和0.01）;治疗8周后A组与治疗4周时比较继续上升（P〈0.01）,而B组上升不明显,2组间比较差异有显著性意义（P〈0.01）。结论：系统康复治疗能更明显地改善早期及恢复期脑卒中偏瘫患者平衡功能和ADL能力。Objective： To investigate the effect of initiative normalization rehabilitation on equilibrium function and activities of daily living in hemiplegic patients after stroke. Methods： Sixty hemiplegic patients with onset for 2 - 12 weeks were randomly assigned to two groups： initiative normalization rehabilitation group （group A） and simple passive exercise rehabilitation control group （group B）. Both of the two groups had the same routine medical treatment. All of them were evaluated by the Berg Balance Scale and functional independence measure （FIM） at the time of beginning, 4th and and 8th week after treatment. Results： In group A, the Berg score at 4th and 8th week was 20. 00±12. 91 and 33.38±15.22, respectively （P〈0.01）. In group B, the Berg score at 4th and 8th week was 416.19±12.13 and 17.58±10.58 respectively. In group A, the FIM score at 4th and 8th week was 84.81±20.78 and 94.38±21.37 respectively （P〈0.01）. In group B, the FIM score at 4th and 8th week was 60.12±10.11 and 65.31±8.61 respectively. The Berg score and the FIM score in group A were obviously greater than in group B （P〈0.01）. Furthermore, from the second month of treatment, the Berg score in group A was continuously increased, but the score in group B was gradually reduced （P〈0.01）. Conclusion： Initiative normalization rehabilitation can improve the equilibrium function and activities of daily living in hemiplegic patients after stroke more obviously than simple passive exercise rehabilitation.
