摘　　要：目的:观察早期肠内营养(EEN)对老年食管贲门癌患者术后恢复的影响。方法:随机将98例老年食管贲门癌术后患者分为EEN组45例,施行EEN治疗;N-EEN组53例,按常规治疗。分别对2组术后并发症的发生率、肠功能恢复时间以及血清免疫学指标进行统计分析。结果:术后并发症发生率EEN组明显低于N-EEN组(分别为5例占11.1%,11例占20.7%,P<0.01);血清IgAI、gG、IgM、清蛋白、前清蛋白、转铁蛋白在术后第9天EEN组较N-EEN组明显升高(P<0.05);术后排气、排便时间EEN组明显早于N-EEN组(P<0.05),所用营养费用低于N-EEN组;肝肾功能、血糖指数2组术后均差异无显著性意义。结论:EEN能明显提高老年食管贲门癌患者术后的免疫机能,显著改善术后的营养不良状况,减少并发症,促进机体康复。Objective： To observe the effects of early enteral nutrition （EEN） on postoperative recovery in elderly patients with esophagocardial carcinoma. Methods： Ninety-eight elderly patients with esophagocardial carcinoma were randomly divided into two groups. Patients in experimental group were subjected to EEN （n=45）, and those in control group to non-early enteral nutrition （N-EEN, n= 53）. The postoperative complications were recorded. The immunoglobulin A （IgA）, immunoglobulin G （IgG）, immunoglobulin M （IgM）, serum albumin, prealbumin, transferrin, alanine aminotransferase （ALT）, bilirubin, creatinine and glucose in two groups were determined on postoperative day 1, 5 and 9, respectively. Results： The total complications both occurred in 16 patients, including 5 in EEN group and 11 in N- EEN group. In EEN group, the IgA, IgG, IgM, serum albumin, prealbumin and transferrin were significantly higher than in N-EEN group on postoperative day 5 and 9, respectively （P〈0.05）. There was no significant difference in serum ALT, bilirubin, creatinine and glucose between two groups. Conclusion： EEN can significantly improve immune function and nutritional status, decrease the incidence of postoperative complications, and accelerate the recovery of postoperative elderly patients with esophagocardial carcinoma.
