摘 要:炎症性肠病(IBD,包括克罗恩病和溃疡性结肠炎)是一种非特异性肠道炎症性疾病,发病率近年有所上升.IBD病因学研究一直是该领域的研究重点,其发病与遗传、免疫、环境因素密切相关,免疫调节紊乱是其发病的关键因素.克罗恩病是Th_1介导的肠道免疫反应.近几年来,越来越多的研究显示,IL-23在Th_1介导的免疫反应中起重要作用.因此,研究IL-23及其受体IL-23受体(IL-23R)与克罗恩病之间的关系有重要意义.Inflammatory bowel disease (IBD) is a chronic inflammatory condition of the gastrointestinal tract and clinically presents either as Crohn's disease (CD) or as ulcerative colitis (UC). The incidence of IBD has increased in recent years and there is an increased risk for developing IBD in cohorts with a higher socioeconomic class. The underlying cause for the disease remains unknown. Studies have shown that IBD is associated with environmental, genetic and immunological factors. Immunoregulation disorder lays a key role in the development of IBD. IBD is a bacteria and cytokine-driven pathologic immune response and CD is mainly involved in the immune reaction mediated by type 1 helper T-cells (Thl). IL-23 plays an important role in the immune reaction mediated by Thl cells. In this paper, we review the molecular immunologic pathogenesis of IL-23 and IL-23R in CD for a better understanding of the etiology of and the therapy for IBD.
