螺旋CT对结肠癌术前诊断和分期的价值

摘　　要：目的探讨螺旋CT在结肠癌术前诊断和分期中的价值。资料与方法回顾性分析47例经手术病理证实的结肠癌患者的螺旋CT表现,并与术后病理结果对照,分析螺旋CT诊断与分期的准确性。结果47例中CT显示肠壁增厚47例、肠腔内肿块39例、肠腔狭窄和形态不规则35例、浆膜面模糊26例、肠系膜索条状或块状软组织影21例。CT发现淋巴结转移35例(直径≥10mm),病理证实31例。12例无淋巴结转移者中,术后病理发现3例有淋巴结转移,敏感性和特异性分别为91.2%和69.2%,总准确性为85.1%。按照改良Dukes分期对照术前与术后病理,38例分期正确,9例分期错误。总的CT分期准确性为80.9%。结论螺旋CT检查对进展期结肠癌诊断和分期具有一定价值。Objective To discuss the value of preoperative spiral CT in diagnosis and staging of colon cancer. Materials and Metheds CT findings of 47 cases with colon cancer confirmed by pathology were analyzed retrospectively and compared with pathological resuhs. Results Spiral CT images showed wall thickening of colon in all 47 cases, intraluminal mass in 39 cases, irregular and stenotic lumen in 35 cases, serosa surface invasion in 26 cases, strip or mass in mesentery in 21 cases, and lymphadenopathy （ ≥ 10 mm） in 35 cases, confirmed invasion in 31 cases by pathology, there were 12 cases without lymphadenopathy, confirmed invasion in 3 cases by pathological results, sensitivity and specificity were 91.2% and 69.2% respectively. Accuracy was 85.1%. Compare with pathological results, there were correct in 38 cases, incorrect in 9 cases. The total accuracy of staging of colon cancer by spiral CT was 80.9% according to improved Dukes staging. CondusionThe preoperative spiral CT is useful in diagnosis and staging of advanced colon cancer.
