肺癌患者外周血Th1/Th2水平测定及其临床意义

摘　　要：目的探讨肺癌患者外周血辅助性T淋巴细胞亚群1(Th1)、辅助性T淋巴细胞亚群2(Th2)失衡与临床病理改变的关系。方法以白细胞介素2(IL-2)和血清γ干扰素(IFN-γ)作为Th1细胞因子,白细胞介素4(IL-4)、白细胞介素10(IL-10)作为Th2细胞因子。应用流式细胞技术分别对45例肺癌患者和30名正常对照者外周血淋巴细胞中Th1和Th2细胞因子的表达水平进行荧光免疫检测,同时分析Th1/Th2失衡与临床病理改变的关系。结果45例肺癌患者外周血中CD4+T淋巴细胞、Th1细胞和Th2细胞的水平分别为(1.0±0.31)%、(7.3±2.1)%和(5.1±2.2)%;正常对照者外周血中CD4+T淋巴细胞、Th1细胞和Th2细胞的水平分别为(1.9±0.11)%、(14.9±5.5)%和(3.3±1.6)%,两组比较差异有统计学意义(t=2.732、2.800、2.744,均P<0.01)。随着肿瘤的不断进展,Ⅲ～Ⅳ期肺癌患者Th1细胞的水平(5.1±1.5)%明显低于Ⅰ～Ⅱ期肺癌患者(8.3±1.8)%(P<0.05),Ⅲ～Ⅳ期肺癌患者的Th2细胞水平(6.6±1.7)%高于Ⅰ～Ⅱ期肺癌患者(4.4±2.1)%(P<0.05)。肺癌伴淋巴结转移患者Th2细胞的水平高于不伴淋巴结转移者(t=2.711,P<0.01);而Th1细胞的水平却明显低于不伴淋巴结转移者(t=2.931,P<0.01)。Th1和Th2细胞的水平在不同年龄、性别、肿瘤大小、病理类型和组织分化程度患者之间差异无统计学意义(均P>0.05)。结论检测肺癌患者外周血Th1、Th2表达对判断病情、了解机体的免疫功能具有重要的临床意义。Objective To evaluate the correlation of Th1/Th2 cytokine levels in peripheral blood with clinicopa-thological features of patients with lung cancer. Methods The expression of Thl cytokines （IL-2, IFN-γ） and Th2 cytokines （IL-4, IL-10） in peripheral blood lymphocyte cells from 45 patients with lung cancer and 30 normal donors were detected by flow cytometry. Results The levels of CD4^＋ T lymphocyte cells, Thl cells and Th2 cells in peripheral blood of patients with lung cancer were 1.0±0.31, 7.3±2.1 and 5.1±2.2,respectively; while those from normal controls were 1.9±0.11, 14.9±5.5 and 3.3 ±1.6 （P〈0.01）. There were statistical differences in Th1/Th2 cells between patients with stage Ⅲ/Ⅳ and those with stage Ⅰ/Ⅱ （P〈0.01）. Lung cancer with lymph node metastasis showed lower levels of Thl cells than those without lymph node metastasis; while showed higher levels of Th2 cells than those without lymph node metastasis. Th1/Th2 cells were not related to age, gender, tumor size, histological types and histopathological grade （P〉0.05）. Conclusion Patients with lung cancer demonstrate an imbalance state of Th1/Th2 cells in peripheral blood.
