摘  要:目的:研究原发性肝细胞肝癌(HCC)中移动相关蛋白-1(MRP-1,CD9)的表达特征,探讨其对HCC发生发展的影响及其意义。方法:采用免疫组化(S-P)法检测90例HCC患者CD9的表达。采用卡方检验、Spearman相关分析、有序结果的逐步累积比Logistic回归方法建立与HCC临床分期(X1)、病理分级(X2)、血清AFP水平(X3)、转移(X4)和大小(X5)有关的方程。结果:(1)HCC中CD9阳性以胞浆为主。(2)HCC中CD9的表达在癌旁强阳性,癌细胞中显著减弱(P=0.0000),并与病理分级(r率=-0.3355和r强=-0.2117)呈负相关,且随转移(r=-0.2279)和血清AFP水平(r=-0.2326)增高表达减少,尤以转移因素为著;Logistic回归后退法方程LogitPj=αj-0.3902X2-0.6732X3-1.0165X4,j=1、2、3,α1=-2.1410,α2=-1.5806,α3=-1.3861。(3)HCC中CD9阳性在单因素时与肿瘤大小负相关,但在多因素时被掩盖。结论:(1)CD9的表达与HCC的分级分化有关,低表达可能是HCC诊断和病理分级的有用指标。(2)CD9的低表达有利于HCC的转移,是HCC预后判断较重要的指标。(3)单因素分析和多因素分析的联合使用对肝癌的检测和机制探讨具有重要价值。To the study the expressive characters of motility-related protein-1 (MRP-1, CD9) in primary hepatocellular carcinoma (HCC) and investigate it's effect on the occurrence and development of HCC. Methods: By immunohistochemistry techniques S-P, the expression of CD9 was detected in 90 eases of HCC. The Logistic regression equations were established among CD9,HCC clinical stage(X1),pathological grading(X2),AFP level(X3),metastasis(X4), and tumor size (X5) with monofactoral analysis and multifaetoral analysis through ordinal stepwise Logistic regression methods. Results: It was found that CD9-positive cells mainly in the cytoplasm of HCC. The expression of CD9 was significantly positive in periearcinoma cells than that of cancer cells of HCC (P=0.000 0). The expression of CD9 was negatively related to pathological grade and stage in HCC. Finaly patients with low expression of CD9 had high level of AFP (r=-0.232 6)and metastasis (r=-0.227 9). The logistic equation was logitPj=aj-0.390 2 X2-0.673 2 X3-l.016 5 X4,j= 1,2,3.al =-2.141 0, a2=-1.580 6, a3 =-1.386 1. The expression of CD9 was negatively related to tumor size in monofaetoral analysis, but disappearing in multifactoral analysis. Conclusion: The expression of CD9 was related to tumor grade and differentiated degree, the low-expression of CD9 may be a used factor on the diagnosis and pathological grade of HCC. The low-expression of CD9 was correlated to metastasis,which maybe a major factor on valuating prognosis of HCC. It is important to detect mechanism of HCC by using both monofactoral analysis and muhifaetoral analysis.
