应用Pendulum矫治器辅助治疗安氏Ⅲ类错(的研究

摘　　要：目的评估应用 Pendulum 矫治器辅助治疗安氏Ⅲ类错的治疗效果。方法选择上颌轻度发育不足且牙列拥挤的安氏Ⅲ类错患者10例(男6例,女4例,年龄10±1.2岁),先使用前方牵引6个月促上颌骨发育,再使用 Pendulum 矫治器治疗6个月推磨牙向后,提供间隙供牙列排齐以及获得磨牙中性关系,最后用全口固定矫治器完成治疗。对 Pendulum 治疗前后以及患者整个正畸治疗前后的 X 线头颅侧位片进行测量分析。结果使用 Pendulum 矫治器治疗后,上切牙轻度前倾前移,U1-SN 平均增大1.98°,PTV-U1平均前移1.01 mm;上颌第一磨牙远中倾斜远中移动,U6-SN 平均减小6.21°,PTV-U6平均后移3.88 mm。与正畸前后相关数据比较,Pendulum 矫治器对前牙移动以及骨性影响不大。结论Pendulum 矫治器能有效辅助上颌发育不足且牙列拥挤的安氏Ⅲ类错的治疗。Objective To evaluate the assistant treatment effect of Pendulum appliance in class Ⅲ malocclusion. Methods Ten （6 boys and 4 girls, age 10±1 years）subjects with deficient maxilla and crowded upper dentition were chosen. Use Face-mask first to accelerate growth of maxilla for 6 months. Pendulum appliance was applied for distal movement of molars for 6 months before using fixed appliance, enough space was available for aligning upper dentition and the molars relationship was ameliorated. Initial and follow-up cephalograms were compared. Results After the treatment of Pendulum appliance, the upper incisors proclined longitudinally, the mean of U1 - SN angle and PTV- U1 increased 1.98 degree and 1.01mm.The upper first molars retroclined and moved distally, the mean of U6 - SN angle and PTV- U6 decreased 6. 21 degree and 3. 88 mm. Pendulum appliance has a little effect on the maxilla and the movement of anterior teeth. Conclusions Pendulum appliance can be an effective modality to assistant fixed appliance on treatment of class Ⅲ malocclusion with the deficient maxilla and crowded upper dentition.
