宫颈鳞癌放疗前后病理组织学、细胞凋亡、增殖细胞核抗原及p53的变化

摘　　要：目的探讨放疗前后宫颈鳞癌组织细胞凋亡情况以及p53蛋白的表达对宫颈鳞癌组织细胞凋亡的影响。方法32例放疗前、后的宫颈鳞癌活检标本,分别进行凋亡细胞的原位检测(TUNEL)、增殖细胞核抗原(PC-NA)、p53蛋白的免疫组化染色检查。结果宫颈鳞癌放疗后细胞凋亡指数明显高于放疗前(26.63±0.37 vs.11.03±0.33,P<0.01);高增殖能力组细胞凋亡指数明显高于低增殖能力组(20.00±15.56 vs.5.21±3.79,P<0.05)。放疗后的PCNA表达指数显著低于放疗前(31.00±10.63 vs.78.50±6.30,P<0.01)。放疗前后p53染色阳性率差异无显著性(P>0.05);p53阳性组细胞凋亡指数与p53阴性组差异无显著性(P>0.05)。结论宫颈鳞癌组织中细胞增殖活跃则自发性凋亡相应增多;放疗后PCNA指数显著降低,而凋亡指数显著增高。Objective To investigate the apoptosis of cervical squamous - cell carcinoma cells before and after radiotherapy as well as the impact of p53 expression to the cells. Methods Biopsy cervical samples taken before and after radiotherapy in 32 patients with cervical squamous - cell carcinoma underwent apoptosis examination by in - situ TUNEL, PCNA and p53 staining by immunohistoehemistry, respectively. Results The apoptotic index in cervical samples taken after radiotherapy was significantly higher than those taken before radiotherapy （ 26.63 ± 0. 37 vs. 11.03 ± 0. 33, P 〈 0.01 ）. Apoptotic index of cell with high proliferative activity was significantly higher than that of cells with low proliferative activity （20.00 ± 15.56 vs. 5.21 ± 3.79, P 〈 0.05 ）. PCNA index after radiotherapy was significantly lower than before radiotherapy （31.00 ± 10.63 vs. 78.50 ±6.30, P 〈0.01 ）. There were no significant differences in the p53 expression rate between cervical samples taken before and after radiotherapy （ P 〉 0.05 ）. No significant differences were observed in apoptotic index between p53 positive group and p53 negative group （ P 〉 0.05）. Conclusion Of the tissues in cervical squamous - cell carcinoma, cells with high proliferative activity are apt to undergo spontaneous apoptosis; PCNA index was significantly lower following radiotherapy with a remarkable increase in apoptotic index.
