液基细胞学和电子阴道镜在宫颈癌筛查中的效果

摘　　要：[目的]评价我国河南省农村地区使用液基细胞学和阴道镜筛查宫颈癌的效果。[方法]对河南省新密市20～54岁农村女性开展以人群为基础的宫颈癌横断面调查。对妇女进行阴道镜检查,收集并分析宫颈脱落细胞标本和活组织标本检查结果。[结果]细胞学检测灵敏度为70.0%,特异度97.9%,阳性预测值28.0%,阴性预测值99.6%。阴道镜检测灵敏度为70.0%,特异度94.4%,阳性预测值12.5%,阴性预测值99.6%。细胞学和阴道镜检测的约登指数分别是67.9%和69.6%,粗一致率分别97.6%和94.1%。[结论]液基细胞学和阴道镜检查对于宫颈癌筛查灵敏度和特异度都较高。[Purpose] To evaluate the screening effects of liquid-based cytologic test （LCT） and colposcopy for cervical cancer. [Methods] A population based cross-sectional study on women aged 20~54 with cervical cancer in Xinmi city,Henan province undergoing,colposcopy,cervical cytology and biopsy examination were analyzed. [Results] For LCT,the sensitivity,specificity,positive predictive value and negative predictive value were 70.0%,97.9%,28.0% and 99.6%,respectively. For colposcopy,the sensitivity,specificity,positive predictive value and negative predictive value were 70.0%,94.4%,12.5% and 99.6%,respectively. Youden＇s indexes for LCT and colposcopy were 67.9% and 64.4%,respectively. Crude agreement for LCT and colposcopy were 97.6% and 94.1%,respectively. [Conclusion] Both LCT and colposcopy have high sensitivity and high specificity for cervical cancer screening.
