义齿基托布源器治疗颌周恶性肿瘤的初步应用

摘　　要：目的:探讨含碘-125粒子义齿基托布源器在颌骨周围恶性肿瘤治疗中的初步应用,并评价其初期疗效。方法:8例颌周恶性肿瘤患者(其中3例鳞癌,3例腺癌,2例黏液表皮样癌),接受手术结合放射性粒子治疗或者单纯放射性粒子治疗,并佩戴义齿基托布源器。患者定期复查并填写生活质量调查表。结果:随访9～31个月(中位时间为12月),7例接受放射性粒子结合手术治疗患者局部均无复发;1例接受单纯放射性粒子治疗者,肿瘤缩小92%。生活质量调查显示患者的生活质量较治疗前提高。结论:含碘-125粒子义齿基托布源器为综合治疗颌骨周围的恶性肿瘤提供了一种可行的方法。Objective：To study the application of dental applicator containing 125I to treat malignant tumors around jaw.Methods：8 patients who had been treated in Peking university hospital of stomatology since 2004.All patients had pathology diagnosis before implantation.There were 3 squamous cell carcinomas of gingiva,3 adenoid cystic carcinomas and 2 mucousepidermal carcinomas of hard palate.Before implantation,all patients accepted CT examination.Then the plan through TPS was made.Only three patients received intertissual 125I radioactive seeds implantation under local anesthesia.All patients wore the dental applicators.All patients accepted CT examination in one week,2 months and 6 months after implantation.Patients were asked to answer a form about the life quality during treatment period.Results：After 9～31 months（median 12 months）,the malignant tumor of the patient who just received brachytherapy reduced 92% in volume,and the pain was significantly decreased.Malignant tumors of other patients didn＇t relapse.Three patients had ulcers in mucosa of oral cavity.All patients didn＇ t have other complications.Furthermore the quality of life was improved.Conclusion：The brachytherapy of dental applicator containing iodine-125 seeds is a feasible way to treat malignant tumors of gingival and hard palate,and it is an effective and convenient method in combined therapy to treat malignant tumors of tissues around jaws,with the advantage of preserving jaw＇s functions and reducing complications.
