直肠癌术后髂内动脉置泵区域性化疗并发症分析

摘　　要：目的:探讨直肠癌术后髂内动脉置泵区域性化疗的并发症发生原因及相应的预防措施。方法:回顾分析直肠癌患者行根治术的同时加行髂内动脉置泵(单侧)化疗发生并发症患者的临床资料。结果:7例发生并发症,其中置泵侧下肢出现不同程度的疼痛和麻木2例,化疗药渗入皮下致皮下组织部分坏死1例,因化疗泵栓塞灌注失败2例,异物反应重第6次灌注后取出1例,灌注后出现畏寒、发热1例。结论:区域性化疗的并发症主要与置管位置有关,但不能排除与化疗药浓度、量、间隔时间、注药速度及个体差异有关,另外还与无菌操作不严格、肝素钠的浓度及术者经验等均有很大关系。Objective ：To evaluate the causes of complication of the regivnal chemotherapy of rectal cancer and preventive measure. Methods;Fifty-eight patients underwent radical operation and unilateral internal iliac artery implanted pump infusion chemotherapy. Results：Of 7 patients with compliation ,2 patients had different degree of pain and numbness, 1 patient had hypodermic necrosis caused by hypodermic drug permeation ,2 patients failed in infusion because of pump embolism, 1 patient had the pump taken out after the sixth infusion chemotherapy because of severe reaction for the foreign body, 1 patient had chill and fever after infusion. Conclusions： Complications of localization chemotherapy is related with lengh of implanted pump, consistence of drug, quantity, interval, infusion space and individual difference as well as relaxed asepsis, consistence of heparin, and experience of operator.
