摘  要:目的:探讨食管早期癌和癌前病变组织中HIF-1α、Bax和Survivin的表达状况与光动力学(PDT)疗效的关系。方法:利用免疫组织化学方法检测32例食管早期癌(PDT治疗后无效(NR)20例,完全缓解(CR)12例)和52例食管癌前病变(PDT治疗后NR19例,CR33例)患者PDT治疗前病变组织中HIF-1α、Bax和Survivin蛋白的表达;TUNEL法检测细胞凋亡,计算凋亡指数(AI)。结果:食管癌前病变NR组HIF-1α阳性率26.3%(5/19),AI为(2.16±2.43),与CR组的24.2%(8/33)和(3.31±3.22)比较,差异均无统计学意义(P>0.05);食管癌前病变NR组Bax阳性率52.6%(10/19)低于CR组的78.8%(26/33),Survivin阳性率63.2%(12/19)高于CR组的33.3%(11/33)(P均<0.05)。食管早期癌患者NR组HIF-1α阳性率60.0%(12/20)高于CR组的16.7%(2/12),Bax阳性率25.0%(5/20)高于CR组的75.0%(9/12)(P<0.01),AI(1.53±2.34)低于CR组的(4.92±4.43)(P均<0.05);食管早期癌患者NR组Survivin阳性率65.0%(13/20),与CR组的50.0%(6/12)比较,差异无统计学意义(P>0.05)。结论:高表达HIF-1α蛋白的食管早期癌PDT疗效较差,HIF-1α可作为预示食管早期癌患者PDT疗效的指标;高表达Bax蛋白的食管早期癌和食管癌前病变患者PDT疗效较好,是预示食管早期癌和食管癌前病变患者PDT疗效的重要指标;高表达Survivin蛋白的食管癌前病变患者疗效较差,可成为预示食管癌前病变PDT疗效的指标。Aim :To study the relationship between the expression of HIF-1α,Bax and Survivin and the photodynamie therapy (PDT) efficacy in early esophageal cancer and precancerous lesions. Methods: Expressions of HIF-1α, Bax and Survivin as well as apoptosis in 32 cases of early esophageal cancer (12 cases were complete response(CR) and 20 cases were none response(NR) after PDT) and 52 cases of dysplasia ( 19 were NR and 33 cases were CR after PDT) before PDT were tested by Immunohistochemistry and TUNEL, respectively, for analyzing their effects on photodynamic therapy efficiency. Results: The positive rate of HIF-1α were 26.3 % ( 5/19 ), and apoptosis index (AI) were ( 2. 16 ± 2.43 ) of NR group with dysplasia, which didnt differ from 24.2% ( 8/33 ) and ( 3.31 ± 3.22) of CR group with dysplasia( P 〉 0.05 ). The positive rate of Bax of NR group with dysplasia were 52.6% (10/19) , higher than 78.8% (26/33) of CR group with dysplasia, and the positive rate of Survivin was 63.2% (12/19) , higher than 33.3% ( 11/33 ) of CR group with dysplasia (P 〈 0.05 ). The positive rates of HIF-1α and Bax were 60.0% (12/20)and 25.0% (5/20), respectively, higher than 16.7% (2/12) and 75.0% (9/12) of CR group with early esophageal cancer( P 〈 0.05 ). AI were ( 1. 53 ± 2.34 ) of NR group with early esophageal cancer, lower than (4.92 ± 4.43 ) of CR group with early esophageal cancer( P 〈 0.05 ). The positive rate of Survivin was 65.0% (13/20) of NR group with early esophageal cancer, which didnt differ from 50.0% (6/ 12) of CR group with early esophageal cancer(P 〉 0.05 ). Conclusion: High expression of HIF-1α in early esophageal cancer correlates with poor PDT outcomes, and HIF-1α could be a prognosis factor of PDT in early esophageal cancer. High expression of Bax in early esophageal cancer and dysplasia correlates with good PDT outcomes. Bax could be an important prognosis factor of PDT in early esophageal
