摘 要:目的:检测人子宫内膜癌(EMC)、增生期内膜和非典型增生内膜组织中磷酸化细胞外信号调节激酶(p-ERK)的活化情况,并探讨其与EMC发生、发展的关系。方法:采用免疫组织化学SP法检测45例EMC组织(患者年龄<45岁15例,≥45岁30例;病理学分级:Ⅰ级18例,Ⅱ级17例,Ⅲ级10例;临床分期:Ⅰ期23例,Ⅱ期16例,Ⅲ期6例;有淋巴结转移5例,无淋巴结转移40例;肌层浸润深度<1/2者21例,≥1/2者24例)、20例非典型增生内膜、10例增生期内膜组织中p-ERK的表达,分析其蛋白表达与EMC的病理学分级、临床分期、淋巴结转移、肿瘤肌层浸润深度以及患者年龄的关系。结果:EMC组织中p-ERK阳性表达率(77.78%)高于非典型增生内膜(35.00%)和增生期内膜(10.00%)组织的阳性表达率(χ2=20.926,P<0.05)。且EMC组织中p-ERK蛋白阳性表达率与病理学分级、临床分期、肌层浸润深度有关(P均<0.05),而与患者年龄、淋巴结转移情况无关(P均>0.05)。结论:ERK的激活与EMC的发生、发展有关。Aim: To detect the expression of phosphorylated extra-cellular signal-regulated kinase(p-ERK) and investigate its relationship with clinicopathological features in endometrial carcinoma(EMC) tissue. Methods:Immunohistochemistry SP method was adopted to detect the expression of p-ERK in specimens of 45 cases of endometrial carcinoma, 20 cases of atypical hyperplasia endomembrane, and 10 cases of estrin phase endomembrane. The relationship between the expressions of p-ERK and their clinicopathological factors such as pathologic grade and advanced clinical stage, lymph node metastasis, infiltrating depth and the age of patients in EMC tissue was also analyzed. The 45 cases of EMC included 18 cases of well differentiated, 17 cases of moderately differentiated,and 10 cases of poorly differentiated of FIGO histology-pathology stage in 1988; 23 cases of clinical stage Ⅰ , 16 cases of stage Ⅱ , and 6 cases of stageⅢ of FIGO surgery-pathology stage in 2000; 5 cases of lymph node metastasis and 40 cases of no lymph node metastasis; 21 cases of superficial myometrial invasion and 24 cases of deep myometrial invasion; 15 cases of younger than 45 years, and 30 cases of older than 45 years. Resuits: Positive staining for p-ERK was observed in 35 of 45 cases(77.78% ) in EMC tissue, which was significantly higher than those in atypical hyperplasia endomembrane( 35.00% ) and estrin phase endomembrane( 10, 00% ) (χ^2 = 20. 926 ,P 〈 0.05 ). The positive p-ERK expression rate was correlated with pathologic grade, clinical stage, and infiltrating depth ( P 〈 0.05 ) , and was not correlated with the age of patients and lymph node metastasis ( P 〉 0.05 ). Conclusion : The activation of ERK may be correlated with the genesis,development, and prognosis of EMC.
