摘　　要：目的观察草酸铂(L-OHP)联合亚叶酸钙(CF),5-氟尿嘧啶(5-Fu)治疗晚期胃癌的近期疗效及毒副反应。方法确诊晚期胃癌患者32例,均为先前行DF、ECF方案治疗失败的患者,治疗方案为:L-OHP85mg/m2ivgtt,h0～2,d1;CF200mg/m2,ivgtt,2h,d1～2;5-Fu400mg/m2,ivbolus2h,d1～2;5-Fu600mg/m2,civgtt,d2～24,d1～2,2周为1个周期,所有患者至少接受3个周期治疗后评价疗效。结果经3个周期以上治疗后,CR1例,PR14例,NC8例,PD9例。总有效率46.9%,主要毒副反应为神经系统毒性、消化道反应、骨髓抑制、肝肾功能轻度损害。结论草酸铂联合亚叶酸钙、5-氟尿嘧啶治疗晚期胃癌疗效较好,不良反应可耐受。Objective To evaluate the efficiency and toxicity of oxalipaltin （L-OHP）based FOLFOX4 regiment chemotherapy in the treatment of advanced gastric cancer.Methods 32 patients diagnosed advanced gastric cancer, which were relapsed after DF/ECF regiment treatment, were enrolled in the study. The FOLFOX4 chemotherapy regimen was comprised of L-OHP 85 mg/m^2 iv gtt, h0-2, d1;CF 200mg/m^2, iv gtt, 2h,d1-2;5-Fu 400 mg/m^2, iv bolus 2h, d1-2;5-Fu 600 mg/m^2,civ gtt d2-24,d1-2. The regimen was repeated every two weeks. All enrolled patients received at least 3 cycles of treatment before estimating effect. Results After 3 cycles of chemotherapy, complete response was achieved in 1 cases, partial response in 14 cases, stableness in 8 cases and progression in 9 cases, The overall response rate was 46.9%. The side effects included peripheral sensitive neuropathy, digestive tract toxin and hone marrow suppression.Conclusion Oxaliplatin based chemotherapy was effective in controlling advanced gastric cancer patients with acceptable toxicity.
