改良乙状窦前入路手术治疗岩斜区胆脂瘤

摘　　要：目的探讨改良的乙状窦前入路手术切除岩斜区胆脂瘤的疗效。方法在尸头上模拟乙状窦前入路并加以改进。将改进的手术入路应用于临床。回顾性分析10例岩斜区胆脂瘤经改进的乙状窦前入路手术治疗的资料。结果胆脂瘤全切除10例;脑脊液耳漏1例,3d后自愈;面瘫加重1例。无手术死亡。结论乙状窦前入路是切除岩斜区胆脂瘤的有效手术入路。本组病例所采用的改良乙状窦前入路创伤小、并发症少、操作简便、易掌握。Objective To explore the therapeutic effect of the modified presigmoid approach for petroclival cholestatoma. Methods Simulative presigmoid approach was operated and modified in adaveric adult heads first,and this modified presigmoid approach was applied in clinic. 10 cases of cholestatomas resected by this approach were retrospectively analyzed. Results Total removal of the cholestatomas was achieved in 10 patients. Cerebrospinal fluid leak occurred in 1 case ,but recovered spontaneously after 3 days. Facial paralysis of 1 case developed. No one died. Conclusion The modified presigmoid approach for petroclival cholestatoma is op- erated and mastered easily ,furthermore ,it has less injury and complication.
