腹腔镜下宫颈癌根治术中阴道延长术的临床应用

摘　　要：目的探讨腹腔镜下施行腹膜阴道延长术在宫颈癌治疗中的作用。方法2003年1月～2006年4月对12例宫颈癌患者实施腹腔镜下宫颈癌根治术,其中6例腹腔镜下腹膜阴道延长术为研究组,另外6例常规腹腔镜下宫颈癌根治术为对照组,比较两组术中及术后情况。结果两组在手术时间、术中出血量和术后恢复时间等方面差异均无显著性,而术后6个月测定阴道长度,两组差异有显著性,研究组阴道深7～10cm,平均7.8cm,对照组阴道深4～5cm,平均4.5cm,且研究组男女对性生活的满意度均高于对照组。结论腹腔镜下宫颈癌根治术中施行腹膜阴道延长手术既达到了微创手术的目的,又有效地改善患者术后生活质量,具有较高的临床应用价值。[Objective] To evaluate the clinincal effects of vagina-lengthening on peritoneum in treatment of cervix cancer by laparoscope. [Methods] 12 cases of cervix cancer had laparodcopic surgery from January 2003 to April 2006 were analyzed retrospectively. 6 cases in trial group had vagina-Lengthening on peritoneum by laparoscope, and 6 cases in control group had radical operation of uterine cervix cancer. The intraoperative and postoperative conditions were compared. [Results] The operation time, volume of bleeding and duration of recovery were similar in both groups. 6 months after operation, the vaginal length of the two groups showed statistical difference： the vaginal length of the trial group was 7-10 cm （7.8 cm on average）, while that in control group was 4- 5 cm （4.5 cm on average）. The satisfaction of sexual life of the trial group was higher than the control group. [Conclusion] Laparoscopic radical correction of uterine cervix cancer, vagina-lengthening on peritoneum limits the incision, and improves the sexual quality of the patients after operations. It shows high clinical value.
