完全腹腔镜治疗胰岛细胞瘤的临床应用及分析

摘　　要：目的探讨腹腔镜胰岛细胞瘤切除的可行性。方法回顾2005年3月～2006年6月间,该科对3例胰岛细胞瘤患者实施的腹腔镜胰岛细胞瘤切除术,手术种类包括:胰岛细胞瘤剜除术2例,胰体尾加脾切除术1例。并结合国内外有关文献报道进行诊治分析。结果手术过程顺利,肿瘤完整切除,3例均在全腹腔镜下完成,术中出血分别为15、70和500mL,2例术后第2天进食、下床活动,1例少量渗出。3例患者均痊愈出院。结论腹腔镜对于胰岛细胞瘤切除是微创、安全、可行的,具有术中出血少、创伤轻、术后恢复快、并发症少等优点,值得进一步推广。[Objective] To discuss the feasibility of the laparoscopic resection of insulinoma. [Method] The data of 3 insulinoma patients who had laparoscopic resections of insulinoma during Mar. 2005 and Jun. 2006 was reviewed. The three laparoscopic resections of insulinoma consisted of 2 cases by gouging out the insulinoma and the 1 case of pancreas body and afterbody and lienectomy. And analysis of these operations was done by reading domestic and overseas papers. [Result] The operations were all successful, the tumors were cut off completely. The three operations were all accomplished by laparoscopy. The hemorrhages during operations were 15, 70, and 500 mL respectively. 2 patients ate and walked after the operations in 2 days. And 1 patient transuded a little. The 3 patients were all healed. [Conclusion] The laparoscopic resection of insulinoma is tiny-wound, safe, and feasible approach. It has the merits such as a little while hemorrhage, slight wound, quick recovery and few complications and so on. So it worth extending.
