摘 要:目的从肿瘤的临床病理学和分子病理学角度对结直肠癌组织中MMP-2的表达与肿瘤外周血微转移的关系进行探讨。方法收集了2003年6~12月行根治手术的结直肠癌患者中的30例,行MMP-2免疫组化染色并应用RT-PCR方法检测外周血的转移肿瘤细胞,比较不同MMP-2表达量与血液微转移的关系。结果76.67%结直肠癌患者的肿瘤细胞表达MMP-2(23/30),定位于胞浆,其中弱阳性13例,阳性4例,强阳性6例;MMP-2的表达与肿瘤分期、淋巴转移相关(P〈0.05)。外周血CK20 mRNA表达率为36.7%,外周血CK20 mRNA与结直肠癌分期相关(P=0.004),与肿瘤部位,大体类型,恶性程度无关。结直肠癌组织中MMP-2的表达与结直肠癌患者外周血CK20 mRNA密切相关,随着MMP-2表达的增强外周血CK20 mRNA的阳性率也增加(P=0.002)。结论结直肠癌组织中MMP-2表达的程度不仅能够反映肿瘤的分期,而且能够反映肿瘤血液中微转移的情况。Objective We discuss the relation between MMP-2 and peripheral blood micrometastasis in colorectal cancer patients from clinical pathology and molecular-pathology. Methods 30 colorectal cancer patients being curative surgical procedure from June 2003 to December 2003 were collected. All cases had been done immunohistochemistry stain with MMP-2 and RToPCR to find micrometastasised tumor cells in peripheral blood, and compare different degree MMP-2 stain with micrometastasised tumor cell in peripheral blood.Results 76.67% patients' tumor cell of colorectal cancer expressed MMP-2 protein(23/30), which located in cytoplasma; in all 30 colorectal cancer patients, 7 were negative, 13 were positive, 4 were slight positive, 6 were strong positive. The expression of MMP-2 was relative with stage of cancer, the lymph metastasis, but not relative with the position of the tumor and pathology type. In peripheral blood, the positive rate of CK20 mRNA were 36.7%, which was relative with stage of cancer and not relative with the position of the tumor, pathology type and the degree of malignance. The expression of MMP-2 protein was relative with the expression of CK20 mRNA in peripheral blood; the more MMP-2 protein, the more micrometastasis in peripheral blood in colorectal cancer patient.Conclusion MMP-2 expression in colocrectal cancer by using immunohistochemistry technology not only can manifest the tumor stage, but also can reflect the condition of the tumor micrometastasis in peripheral blood.
