β-内酰胺酶抑制剂tazobactam与舒巴坦的比较研究

摘　　要：用琼脂二倍稀释法研究了ｔａｚｏｂａｃｔａｍ、舒巴坦与头孢哌酮、哌拉西林、头孢唑林、氨苄西林对６６株双重耐药菌的体外协同抗菌作用。研究结果表明：除铜绿假单胞菌外，ｔａｚｏ－ｂａｃ－ｔａｍ、舒巴坦与４种β－内酰胺类抗生素均有不同程度的协同增效作用。其中ｔａｚｏｂａｃｔａｍ与头孢哌酮的协同作用最强，可使头孢哌酮对大肠杆菌的ＭＩＣ５０从３２ｍｇ／Ｌ降至０．５ｍｇ／Ｌ。头孢哌酮与舒巴坦、哌拉西林与ｔａｚｏｂａｃｔａｍ的协同作用其次，氨苄西林与舒巴坦的协同作用较弱。并且随着ｔａｚｏｂａｃｔａｍ的浓度增大，其抑制作用逐渐增强。The in vitro activities of the β lactamase inhibitors tazobactam and sulbactam combined with cefoperazone, piperacillin, cefazolin, and ampicillin against Gram negative resistant bacteria selected from clinics were studied. Results showed that tazobactam could enhance the antibacterial activities of 4 β lactams against 15 strains of E.coli , 10 strains of E.cloacae , 9 strains of C.diversus , 6 strains of S.marcescens , 6 strains of K.pneumoniae , 3 strains of E.aerogenes , 3 strains of Salmonella .spp and 14 strains of P.aeruginosa . Among the combination of tazobactam and sulbac tam with 4 β lactams, except P.aeruginosa , the syngerism of cefoperazone and tazobactam was the strongest. When tazobactam added, the MIC 50 of cefoperazone against E.coli was reduced from 32mg/L to 0.5mg/L (cefoperazone/tazobactam). The syngerism of piperacillin and tazobactam was inferior to that of cefoperazone and tazobactam, cefoperazone and sulbactam, and superior to that of ampicillin combined with sulbactam. When the concentration of tazobactam was increased, the inhibitory activity of it increased.
