摘 要:目的应用组织芯片免疫组化技术探讨粘着斑激酶(FAK)表达与喉和下咽鳞状细胞癌生物学行为的相关性。方法设计制作喉及下咽鳞状细胞癌组织芯片,应用免疫组化技术在组织芯片上检测FAK的表达,分析FAK表达与喉及下咽鳞状细胞癌临床和病理分期的相关性。结果FAK在原发癌中的阳性表达率为96.00%(72/75),明显高于癌旁正常组织(13/29,44.83%,P<0.01);淋巴结转移癌中FAK的阳性表达率为100.00%(20/20),明显高于癌旁正常组织(P<0.001);有淋巴结转移组FAK表达率为95.35%(41/43),明显高于无淋巴结转移组(12/32,37.50%,P<0.01);高分化组FAK表达率明显低于中、低分化组(P<0.001);而FAK的表达率与肿瘤的原发部位、浸润范围(T分期)、年龄和性别等因素均无明显相关性(P>0.05)。结论FAK在喉癌和下咽癌组织中高表达,其表达率与喉癌和下咽癌的分化程度和淋巴结转移有关,可作为临床预测喉癌和下咽癌颈淋巴结转移趋势和估计预后的主要参考指标之一。Objective To investigate the expression of the focal adhesion kinase (FAK) in laryngeal and hypopharyngeal squamous cell carcinomas and analyze the relationship between FAK expression and biological behavior of this cancer by using the immunohistochemistry in tissue microarray. Methods We designed and made a tissue microarray of laryngeal and hypopharyngeal squamous cell carcinoma (SCC), and investigated the expression of FAK in SCC of the larynx and hypopharynx by immunohistochemistry in the tissue microarray, then analyzed the clinical significance of FAK expression in this eancer. Results In primary cancer tissue, the expression rate of FAK was 58.67% (44/75), significantly higher than normal tissue (34.48%, 10/29, P〈 0.05) ; FAK expression rate in lymph node metastatic carcinoma is 95.00% (19/20), obviously higher than normal tissue (P〈0. 001 ) and primary cancer tissue (P 〈 0.01 ) ; FAK expression rate of the group with lymph node metastasis is higher than group without lymph node metastasis (P 〈 0.05) ; the expression rate of the well- differentiated group was higher than moderately and poorly differentiated group (P 〈 0. 001 ). Conclusions FAK is overexpression in SCC of larynx and hypopharynx, the expression rate is concerned with degree of cell differentiation and metastasis of lymph node, it may become a predict index of metastasis and prognosis of the laryngeal and hypopharyngeal squamous cell carcinoma.
