双气囊电子内镜在小肠克罗恩病诊断中的应用

摘　　要：目的:研究双气囊电子内镜(DBE)对小肠克罗恩病(CD)的检出率和确诊率,并与多层螺旋CT小肠造影(MSCTE)进行比较,探讨其在临床实际操作中的价值和应用。方法:对189例临床疑为小肠CD的患者行DBE检查,其中48例患者同时行MSCTE检查,对比分析结果。结果:189例临床疑为小肠CD的患者经DBE检查及后期随访证实了143例,DBE总体病变检出率为171例。DBE对小肠CD的诊断率为75.7%;MSCTE对小肠CD的诊断率为81.5%,近似于DBE。结论:DBE对小肠CD具很高的诊断率,可作为其诊断的标准手段。MSCTE对小肠CD具较高的诊断率,诊断价值近似DBE。Objective To investigate the detection rate and accuracy of double-balloon endoscopy（DBE）in the diagnosis of small intestine Crohn′s disease （CD） and compare it with the results of multi-slice CT enteroclysis （MSCTE）. Methods Double balloon enteroscopy was performed in 189 patients with suspected small intestine Crohn′s disease. Among them 48 also had taken MSCTE. The results of these two examinations were compared and analyzed. Results Of the 189 patients 143 had their diagnosis finally established by DBE and follow-up. Lesions were detected in 171 patients by DBE.The overall diagnostic yield of DBE was 75.7%; that of MSCTE was 81.5%. The diagnostic accuracy rate of DBE is similar to that of MSCTE. Conclusions DBE has a high diagnostic yield and accuracy in small intestine CD and can be regarded as a standard diagnostic modality.MSCTE is a very useful and reliable method for diagnosing small intestine CD.Its diagnostic value is similar to that of DBE.
