摘  要:目的观察无水酒精瘤内注射治疗肝癌的效果。方法50例肝癌患者共79个结节在超声引导下细针穿刺经皮瘤内注射无水酒精,致肿瘤细胞及其血管内皮细胞迅速脱水,蛋白凝固,癌细胞变性坏死。肿瘤直径2.5-10cm,每个病灶每次注射酒精2-6ml,每周2次,4-6次为1疗程。结果50例肝癌患者经治疗后,甲胎蛋白下降率77%,转阴率60%;B超示:51%肿瘤结节直径缩小,平均直径从3.4cm降为2.7cm,2%肿瘤结节消失。肿瘤直径≤3cm组1年和3年生存率分别为87%与76%,高于直径>3cm组的52.5%与42%。结论本疗法操作简单,适应症广,副作用少,疗效确定,患者易于接受,值得基层医院推广使用。Objective To retrospectively review the curative effects of the ethanol injection therapy in the treatment of hepato-cellular carcinoma. Methods 50 patients with 79 nodules were treated with ultrasound-guided fine-needle injection of anhydrous alcohol. As a result, tumor cells and endothelial cells dehydrated rapidly, protein coagulated, and finally cells were degenerated and neerotic. The diameter of the tumors ranges from 2.5 cm to 10 em, each focus was injected with anhydrous alcohol of 2 - 6 ml two arm clasp times of injection per week. Results By using therapy with anhydrous alcohol injection, the rate of decline in alpha - fetoprotein (AFP) levels was 77 %. Ultrasound B - scarl showed that the diameter of 51% of the tumor nodules was contracted averagely from 3.4 cm to 2.7cm and 2% of the tumor nodules disappeared. The one - year and three - year survival rates of patients with the diameter of the tumor nodules smaller than 3cm were 87% and 76% , respectively; the one-year and three -year survival rates of patients with the diameter of the tumor nodules smaller than 3cm were 52.5% and 42% , respectively. Conclusion The method is easy to be accepted by patients because of little side effects and low in expense, and deserves to be popularized in primary hospitals.
