肾透明细胞癌超声造影模式与其病理分化程度的相关性

摘　　要：目的探讨肾透明细胞癌的超声造影(CEUS)模式与病理分化程度的相关性。方法对病理结果为肾透明细胞癌的24例患者的超声造影结果进行回顾性分析,并将其超声造影模式与病理分化程度进行对照研究。结果肾透明细胞癌超声造影模式分四型:型:快进快出型8例,型:快进慢出型5例,型:同进同出型5例,型:慢进慢出型6例,前3型病理组织表现为分化程度较低,血管成分及血管弹力层纤维丰富,间质成分较少;型病理组织表现为分化程度较高,间质成分较多,血管成分较少。结论肾透明细胞癌的造影模式与肿瘤病理分化程度密切相关。Objective To evaluate the relationship between contrast-enhanced ultrasound （CEUS） pattern of renal clear cell carcinoma and tumor pathologic differentiation. Methods Twenty four clear cell carcinoma lesions with pathologic results were examined on pulsed subtraction CEUS with a bolus injection of SonoVue. Using the machine-drawn CnTI （contrast tuned imaging ） and time-intensity curves, the CEUS results were analysed retrospectively. Results Enhancement patterns of renal clear cell carcinoma were divided into four models. Eight cases with type Ⅰ ,renal clear cell carcinoma indicated contrast enhancement patterns of fast in and fast out ;five cases of type Ⅱ ,fast in and slow out;5 cases of type Ⅲ indicated same in and same out. If the pathologic differentiation was low, more vascularity and less mesenchymal tissues were on ultrasound. Six cases of type Ⅳ indicated contrast enhancement patterns of slow in and slow out. In high pathologic differentiated renal clear cell carcinoma, ultrasound revealed less blood vascularization and abundant mesenchymal tissues. Conclusions Contrast enhanced patterns of renal clear cell carcinoma were correlated with tumor pathologic differentiation.
