摘 要:目的 研究棱镜组合式透镜的安全性、适应性以及对青少年近视发展的影响作用。方法招募171名7~13岁广州市近视儿童分为单光镜组89名.棱镜1组(下加+1.5D)40名、棱镜2组(下加+2.0D)42名3组。所有实验对象在配戴初、配戴6个月与配戴12个月后进行电脑验光、散瞳检影、A超眼轴长度测定及眼位测量。结果共有76.6%(131名)完成1年随访。3组人远隐斜改变量依次为(0.23±1.17)△、(0.28±2.35)△、(-0.06±2.24)△(P组间=0.1359,P时间=0.4518)近隐斜改变量依次为(-3.87±4.76)△、(-4.32±5.51)△(-4.47±6.08)△(P时间=0.36,P组1〈0.01)。3组病人屈光度增长数值依次为(-0.75±0.41)D、(-0.64±0.42)D、(-0.48±0.40)D(P〈0.01),组别(P<0.01)与年龄(P=0.01)是具有统计意义的影响因素。眼轴总长增长值依次为(0.48±0.34)mm、(0.22±0.20)mm、(0.15±0.20)mm(P〈0.01),组别(P<0.01)是具有统计意义的影响因素。结论与单光镜相比,下加+2.00D的棱镜组合式透镜在控制近视患者负球镜增长方面有显著性差异,这种差异与眼轴长度相关。下加+1.5D、+2.0D棱镜组合式透镜对近视患者远近隐斜无明显影响,但患者的依从性较单光镜差,棱镜组合式透镜的更长期效果有待进一步观察。Objective To value the safety, adaptability and effectness on myopia progression of Prism Combined with Plus lens(PCPLs). Methods 171 Chinese juvenile-onset myopes in Guangzhou city were enrolled, whom were assigned to the single vision lens (SVLs) group, the PCPLs Ⅰ group(add+1.5D) and PCPLs 2 group(add+2.0D). The primary outcome included myopic progression (determined by autorefraction after cycloplegia), ocular biometry (measured by A-scan ultrasonography) and heterophoria status (determined by Cover Test).Results Of the 171 children, 131 (76.6%) completed the first year visit. The change of Phoria at diatant are (0.23 ± 1.17)△ (0.28 ±2.35)△ (-0.06 ± 2.24)△(pgroup=0.1359, Ptimc=0,4518); The change of Phoria at near are (-3.87 ± 4.76)△ (-4.32 ± 5.51)△ (-4.47 ± 6.08)△ ( Ptimc=0.36, P p〈0.01).The change of SE were (-0.75 + 0.41)D, (-0.64 ± 0.42)D, (-0.48 ± 0.40)D (P〈0.01).The chang of axial length is (0.48 ± 0.34) mm, (0.22 ± 0.20)mm, (0,15+0,20)mm (P〈0.01). Conclusion The In'st year's results show that, compared with SVs, PCPLs can retard myopic progression partly, and the two have same safety quality.The PCPLs adaptability is lower than SVLs. However, the long-term effect of PCPLs needs further observation,
