自体唇黏膜固有层在唇裂手术中的应用

摘　　要：目的观察唇裂手术中,将自体唇黏膜固有层移植于皮肤创口内的可行性和效果。方法自2003年1月至2006年12月,选择年龄为3～5个月的单侧唇裂伴腭裂患儿28例,在下三角瓣法唇裂修复术中,将切下的唇红黏膜制成固有层后,自体移植于患侧鼻槛区的表皮下。结果术后7d,28例患儿的鼻槛区切口无红肿及异常渗出,甲级愈合。术后12～17个月,随访其中的19例患儿,鼻槛区的瘢痕平坦且较窄,肤色与周围皮肤相近;在移植较大固有层的6例患儿中,鼻槛区较丰满、微隆起者4例,较丰满、无隆起者2例。结论在唇裂修复术中,唇黏膜固有层自体移植于皮肤切口内的方法是可行的,并可用于表皮下填充。Objective To explore the effects and feasibility of autograft lip mucosa lamina prorias in skin wounds during cheiloplasty. Methods From January 2003 to December 2006, 28 unilateral cleft lips together with cleft palates in patients of age 3 to 5 months, underwent repair by cheilopasty using the Tennision method. Lamina proprias were constructed from superfluous lip mueosa and were grafted in skin incisions located narial sills. All wounds were observed for 7 days during hospitalizaton. Results In all 28 narial sill wounds, there was no infection through the wound healing process for the 7 days after grafting. After 12- 17 months follow up postoperatively, 19 narial sill wound scars were flat and narrow, and their color was similar to the normal; 6 narial sill wounds which grafted in with bigger lamina proprias were smilar to the first 19 eases, 4 in above 6 narial sill tissues were enough and little prominent, but 2 in above 6 narial sill tissues were only enough. Conclusion It is suggested that autograft of lip mucosa lamina proprias in skin incision is feasi- ble, and the lip mueosa lamina proprias can be effective for filling under the epidermis during cheiloplasty.
