全脑照射20Gy后综合治疗与单纯放射治疗脑转移瘤的临床研究

摘　　要：目的 分析脑转移瘤全脑照射20 Gy后辅以化学治疗与单纯放射治疗脑转移瘤，评价两种治疗方法的疗效和预后因素。方法 回顾性分析经病理学和影像学证实的脑转移瘤患者63例，随机分成两组。综合治疗组33例，予以全脑照射20 Gy后辅以化疗，放疗同步治疗（放疗总剂量DT：35～40Gy，3～4周），并X-刀追量至40-60Gy；对照组30例，单纯放射治疗总量至35～40 Gy，3～4周，并X-刀追量至40～60 Gy。结果 综合治疗组中位生存时间为14个月，1年生存率为85．3％；对照组中位生存时间8个月，1年生存率为40．5％。两组差异有统计学意义（P〈0．05）。结论全脑照射20 Gy打开血脑屏障辅以化学治疗脑转移瘤有较好的疗效和预后。Objective To analyze the clinical research of combined therapy after whole brain irradiation 20 Gy and simple radiation therapy to brain metastatic tumor,appraise the therapeutic effect and prognosis factor for the two therapeutics. Methods Retrospective analysis brain metastatic tumor 63 patients to confirm with pathology and imageology, random fractionate two sets, combined therapy 33 examples, after whole brain irradiation 20 Gy to give chemotherapy and radiotherapy synehron therapy （total dose of radiotherapy DT：35 -40 Gy,3 -4 weeks） ,and X -knife to add the dose up to 40 -60 Gy. Control group 30 examples, simple radio- therapy total dose to 35 - 40 Gy,3 - 4 weeks, and X - knife to add the dose up to 40 - 60 Gy. Results The meso -live time of combined therapy set was 14 months,the 1 year survival rate was 85.3%. The meso -live time of control group was 8 months, the 1 year survival rate was 40.5% o The difference of two sets had statisieal significance （P 〈 0. 05 ） . Conclusion After whole brain irradiation 20 Gy to open blood brain barrier, and combined therapy brain metastatic tumor has the better therapeutic effect and prognosis.
