摘 要:目的:探讨CD44v6与MMP-9在宫颈癌中的表达及生物学意义,验证CD44v6、MMP-9与宫颈癌浸润转移的关系并揭示两种蛋白表达的相关性。方法:建立组织芯片(Tissue Microarry,TMA)技术平台,应用该技术结合免疫组织化学S-P法检测两种蛋白在宫颈癌石蜡包埋组织中的表达。结果:组织芯片制备成功。对其HE切片镜检,157例标本3例取材不准、1例脱片。三种蛋白的组织芯片免疫组化切片脱片率为0.025%。其中CD44v6在正常宫颈(NCE)、宫颈上皮非典型增生(CIN)和宫颈浸润癌(ICC)中的阳性率分别为25.0%、53.5%和78.5%,ICC分别与NCE和CIN比较有统计学意义(P<0.01和P<0.05)。而MMP-9的阳性率分别为21.8%、50.0%和81.7%,其中ICC、CIN分别与NCE比较有统计学意义(P<0.01和P<0.05)。宫颈癌中CD44v6和MMP-9的表达与病理分级有关(P<0.05)。伴有盆腔淋巴结转移组其CD44v6、MMP-9阳性表达率显著增高(P<0.05)。两种蛋白与年龄、临床分期、病理类型等关系不显著。MMP-9与CD44v6在宫颈癌中的表达呈明显正相关(P<0.05)。结论:建立了组织芯片,为研究宫颈癌的生物学行为提供了有力的技术平台。宫颈癌CD44v6和MMP-9表达增强可能使宫颈癌更易发生侵袭转移。因此,联合检测宫颈癌中CD44v6、MMP-9的表达对进一步了解宫颈癌生物学行为和判断其预后有一定的价值。Objective: To investigate the expression and biological signicificance of CD44v6 and MMP - 9 proteins in cervical cancer, verify the relationship among the expression of CD44v6 and MMP - 9 protein and the cervical carcinoma development and metastasis and reveal the correlation of the three proteins expression. Meth- ods: Tissue microarray (TMA) platform was established. The immunostaining of the antibodies about the proteins above - mentioned in the paraffinembedded cervical tissues was performed on the basis of cervical TMA with the immunohistochemistry streptavidin- peroxidase (SP) method. Results: Cervical tissue microarray from 157 patients was completed. By microscopic examination to the HE slide of cervical tissue microarray, we found 2 error cases owing to accurate location and 1 shedding case among 157 cases. The shedding rate of the three TMAs about proteins were 0. 025 %. The positive rate of CD44v6 protein expression were 25.0 % in normal cervical epithelium(NCE), 53.5 % in cervical intraepithelial neoplasis(CIN), and 78.5 % in invasive carcinoma of cervix(ICC). There was significance between ICC and NCE (P 〈 0.01). There was also significance between ICC and CIN(P 〈 0.05). The positive rates of MMP - 9 protein expression were 21.8 % 、 50.0 % and 81.7 % in three types. There was significance between ICC and NCE (P 〈 0.01). There was also significance between ICC and CIN (P 〈 0.05) . In ICC, the expression of CD44v6 and MMP - 9 associated with differentiation. The rate of positive staining reaction of MMP - 9 and CD44v6 was signlicantly higher in the cases with pelvic lymphonode metastasis than those in the cases without metastasis, (P 〈 0.05). By studying the relationship among three proteins in cervical carcinoma, there was obviously positive correlation between the expression of CD44v6 and MMP- 9 (P〈0.01).
