摘 要:目的研究不同b值前列腺癌的扩散加权成像(DWI)表现,比较表观扩散系数(ADC)之间的差异,探讨最佳b值的选取。方法对20例经病理证实的前列腺癌(Pca)患者行DWI扫描,b值分别取0s/mm2,250s/mm2,500s/mm2和750s/mm2,观察前列腺癌不同b值的DWI表现,计算前列腺癌与周围组织之间的对比度,测量癌灶的ADC值并作统计学分析。结果20例前列腺癌在b=0s/mm2时呈低信号影,b=250s/mm2时5例呈低信号影,6例呈等信号影,9例表现为稍高信号影,b值取500s/mm2和750s/mm2时呈高信号影,b值分别取250s/mm2,500s/mm2和750s/mm2时,Pca与周围组织的对比度依次为(6.355±4.261)%,(11.426±5.689)%和(19.528±5.712)%,在不同b值DWI测得前列腺癌与周围组织对比度之间差异有统计学意义(F=31.857,P<0.05);Pca的ADC值依次为(1.463±0.143)×10-3mm2/s,(1.199±0.121)×10-3mm2/s和(0.929±0.120)×10-3mm2/s,不同b值测得前列腺癌ADC值之间差异有统计学意义(F=86.618,P<0.05)。结论750s/mm2是较佳b值,Pca呈高信号影,获得的DWI图像对比度最好,可测得相对准确的ADC值,有助于其定性诊断。Objective To investigate the diffusion weighted imaging (DWI) and the apparent diffusion coefficient (ADC) values of prostate carcinoma (Pea) in different b factors, and to determine the optimal b factor of Pca. Methods 20 patients with proven Pca were scanned with DWI respectively, b factors were set as 0, 250, 500 and 750 s/mm^2. Their DWI and ADC map appearances were analyzed. The ADC values of the Pea foci among different b factors were measured. The contrast between Pea and normal tissues around in different b factors were calculated. Results When b factor was 0 s/mm^2 , all the 20 Pca showed low signal intensity. When b factor was 250 s/mm^2 , 5 eases showed low signal intensity, 6 cases showed iso -signal intensity, and 9 cases showed slight high signal intensity. As b factors were 500 and 750 s/mm2, all the 20 Pca showed high signal intensity. The mean ADCs for the Pca foci were (1. 463 ± 0. 143)× 10^-3 mm^2/s, | 1. 199± 0. 121 )× 10^-3 mm^2/s, and (0. 929 ±0. 120 ) ×10^-3 mm^2/s when b factors were 250, 500 and 750 s/mm^2 respectively ( one -way ANOVE , F = 86. 618, P 〈 0.05 ). The mean contrasts between Pea and surrounding normal tissues were ( 6. 355 ±4. 261 ) % , ( 11. 426± 5. 689)% , and ( 19.528 ±5.712) % when b factors were 250, 500 and 750 s/mm2 respectively ( one -way ANOVE,F =31. 857,P 〈0.05). Conclusion 750 s/mm^2 is the optimal b factor for DWI of Pea. Pea showed high signal intensity on DWI which can be helpful in the qualitative diagnosis.
