多形性未分化肉瘤动态增强MR的诊断价值

摘　　要：目的分析多形性未分化肉瘤动态增强MR表现,以提高其诊断准确性。方法回顾性分析经手术病理证实的多形性未分化肉瘤11例,术前分别经MRSET1WI、FSET2WI,动态增强采用T1WI或GRE,仔细复习MR扫描结果并和手术病理作对照。结果11例多形性未分化肉瘤中,位于四肢5例、躯干3例、腹膜后2例、心脏1例,肿瘤直径4.9～11.8cm,平均7.6cm。MRT1WI为等或略低信号,T2WI为等信号、略高信号和显著高信号等多重信号混杂,实质成分T2WI多为片状等或略高信号;增强扫描11个病例强化显著,呈进行性延迟强化。结论多形性未分化肉瘤具有一定的MR特征。Objective To explore the imaging features of undifferentiated pleomorphic sarcoma of soft tissue so that to improve the knowledge of this disease. Methods 11 cases with undifferentiated pleomorphic sarcoma confirmed by surgical pathology underwent MR SE -T1WI, FSE -T2WI and SE -T1WI enhancement scanning before operation, the imaging date were reviewed and analysed retrospectively in comparison with surgical and pathological results. Results The tumors were located at limbs in 5, trunk in 3 ,retroperitoneum in 2 and left atrium in 1 , respectively. The diameter of the tumours was 4.9 - 11.8 cm with the mean diameter of 7.6 cm. The lesions were iso - or slight low signal intensity on T1WI and mixed iso - , slight hyperintense and high signal intensity on T2WI. On dynamic contrast -enhanced MRI in 11 cases,the tumors were inhomogeneous delayed obvious enhancement. Conclusion The undifferentiated pleomorphic sarcoma is of certain specific MRI features.
