复发性急性胰腺炎的影响因素和预后分析

摘　　要：目的分析引起急性胰腺炎反复发作的影响因素及复发性胰腺炎的预后。方法对我院收集的191例急性胰腺炎根据急性胰腺炎发作次数分为单发组和复发组，对两组的病史特点、临床表现和预后进行分析比较。结果单发组156例（81．7％），复发组35例（18．3％），两组平均年龄无显著差异；男性复发率明显高于女性（P〈0．005），年龄相对集中于21—40岁。单因素分析显示：饮酒、高脂血症病史、血清高三酰甘油（TG）及高血糖与复发性急性胰腺炎（RAP）相关；逐步Logistic回归发现高TG血症、饮酒为RAP最有可能的影响因素。复发组病人发生重症胰腺炎的比例明显高于单发组（P〈0．005）。结论高TG血症及饮酒是急性胰腺炎反复发作重要的影响因素；RAP更容易发生急性重症胰腺炎。Objective To analyze the relative risk factors of recurrent acute pancreatitis and learn more about its prognosis. Methods The data of 191 inpatients with acute pancreatitis in our hospital collected retrospectively were divided into tow groups, simple acute pancreatitis and recurrent acute pancreatitis, and further analyzed the difference of the epidemiology, etiology, clinical characters and assessment of the severity of actue pancreatitis. Results 35 patients （ 18.3% ） with recurrent acute pancreatitis in total 191 acute pancreatits, frequency of male patients was higher than female patients（ P 〈 0. 005 ） and most of recurrent acute patients aged during 21 to 40 years, alcohol, hyperlipidemia（ elevated serum triglyceride level） and hyperglycaemia were associated with recurrent of acute pancreatits in univariate general linear mode analysis. Alcoholan and hyperlipidemia（ elevated serum triglyceride level） may be more possible risk factor of recurrent acute pancrreatitis in step Logistic regression analysis. And the frequency of severe acute pancreatitis was higher in recurrent acute pantients than in simple acute pancreatitis（ P 〈 0.005 ）. Conclusion alcohol and the elevated serum triglyceride level may be important risk factors of recurrent acute pancreatits and severe acute pancreattis more often occur in recurrent acute pancreatits.
