下颌前伸矫治器治疗阻塞性睡眠呼吸暂停低通气综合征效果研究

摘　　要：目的探讨三种不同体位下,下颌前伸矫治器治疗阻塞性睡眠呼吸暂停低通气综合征(OSAHS)的疗效,并分析气道周围软硬组织结构关系的影响。方法10例OSAHS患者在三种不同体位下戴入矫治器前后分别进行多导睡眠仪监测和头影测量分析。结果10例OSAHS患者戴入矫治器后,下颌骨前伸,咽腔结构及周围软组织均发生相应改变。结论下颌前伸矫治器是治疗轻度OSAHS的有效方法,在不同体位下均可改变口咽通气道。Objective To investigate the feasibility of mandibular advancement device for the treatment of obstructive sleep apnea/hypopnea syndrome（OSAHS） and the effects of three body position on the upper airway. Methods Ten patients were diagnosed as having OSAHS by overnight polysomnographic and roentgenographic cephalometric analysis with mandibular advancement device in different postures. Resuits After the treatment with mandibular advancement device, the position of mandibular was changed to protrusive one, with corresponding changes in the shape of pharyngeal cavity and its positional relation with surrounding soft tissues, among all the 10 cases. Conclusions Mandibular advancement device is effective for the treatment of OSAHS. The dimension of airway is changed by mandibular advancement device in three postures.
