摘  要:目的观察内镜下氩离子凝固术(APC)治疗消化道息肉的临床疗效。方法对明确诊断的消化道息肉患者42例进行APC治疗。用DEVEL公司APC9000型氩离子凝固器,电凝功率为60W,氩气流量为2.4L/分钟,至病灶上方0.3~0.5cm处,以每次1~3秒施以氩离子凝固治疗。观察治疗效果及并发症,并于1个月后进行复查。结果42例患者127枚息肉均予摘除/灼除。直径>1cm的广基或带蒂息肉18枚先用圈套器电凝电切摘除,再用APC烧灼残余病灶,直径<1cm的息肉109枚直接用APC烧灼。1个月以后复查胃肠镜有3例(7.1%)直径>1cm的广基或带蒂息肉病灶残端见黏膜充血水肿,未见息肉样增生,再以APC治疗。APC治疗中出现局部疼痛3例,腹胀6例,持续时间1小时~3天,无消化道出血及穿孔发生。结论APC治疗消化道息肉便捷、疗效较好、安全,为消化道内镜治疗提供了广阔空间。Objective To observe the efficacy and safety of endoscopic argon plasma coagulation (APC) in the treatment of gastrointestinal polyps. Methods Forty-two patients diagnosed with gastrointestinal polyps were enrolled. Endoscopic APC was administrated by using APC 9000 with an argon flow of 2. 4 L/rain at a power of 60 W. The therapeutic efficacy and the complications were recorded. Gastroscope was repeated after one month. Results APC was performed in 127 lesions of gastrointestinal polyps, 18 of which with diameter 〉1 cm were treated by polypectomy and followed by APC for the residual lesions, the remaining 109 lesions with diameter 〈1cm were treated by APC only. After 1 month, gastroscope were repeated, residual lesions were only observed in 3 patients(7. 1%)and successfully retreated by APC. The overall complication of the treatment of APC occurred in some cases were recorded as pain at the treatment site and gaseous distention which remained 1 hour to 3 days. There was no major complications occurred such as gastrointestinal bleeding and perforation. Conclusion APC is a simple. safe, effective means in the treatment of gastrointestinal polyps. Providing a wide space in the field of endoscopic therapy.
