66例老年患者种植固定修复的长期临床效果观察

摘　　要：目的评价老年患者种植义齿固定修复的长期临床效果。方法1995年6月至2004年5月,为66例60岁以上老年患者植入了149颗种植体并完成上部结构修复,种植体系统分别为ITI系统和BLB系统。经过19个月至9年的追踪,进行X线片、临床观察、患者满意度调查评估。结果61例患者141颗种植体完成骨整合。5例患者8颗种植体松动脱落,累积存留率94·63%。2例折断;烤瓷冠粘结患者中4例崩瓷,5例冠脱落。结论种植义齿是有效的修复缺失牙的方法,但由于老年患者的生理心理特点,需要给予重视,口腔卫生差引发的感染是老年患者种植失败的主要原因。Objective To evaluate the clinical results of implant fixed demure in aged patients. Methods A total of 149 implants were consecutively implanted in 66 aged patients from 1995 to 2004. The follow-up time was 19 months to 107 months （ average ： 47.3 months ）. Clinical examination, X-ray and patients satisfaction were evaluated. Results Of the 149 implants, 141 were osseointegrated and functioned normally in 61 of 66 patients, and eight implants in 5 cases failed due to infection. The cumulative survival rate was 94. 63%. Prostheses were loose in 11 cases . Ceramic crack was observed in 4 cases and ceramic dropped out in 5 cases. Conclusion Implant-retained fixed denture can be used in aged patients with high survival rate. The long term results were satisfactory.
