透气性硬性角膜接触镜配戴者依从性与安全性的回顾性分析

摘　　要：目的探讨透气性硬性角膜接触镜(RGPCL)配戴者依从性与安全性。方法回顾性分析183例验配RGPCL患者的复查频率,换镜频率及眼表并发症的发生情况;A超和非接触角膜内皮细胞显微镜检查频率,角膜地形图检查频率;戴镜前后角膜内皮细胞及角膜厚度的变化。结果北京患者1周、1个月、3个月、6个月及1年复查率分别为80.2%、72.5%,57.1%,61.5%,57.1%,外地患者为35.4%,43.1%,54.5%,57.0%,54.9%;所有戴镜患者戴镜适应后平均2～3月复查一次,12～15个月换镜一次。并发症主要为角膜轻度点染(Ⅰ级),北京患者22例(12.1%),外地7例(3.8%)。北京患者A超及角膜内皮显微镜6个月检查频率、角膜地形图3个月、6个月检查频率分别为83.5%,86.8%,91.2%,36.3%;外地患者分别为89.9%,92.4%,96.2%,41.8%;戴镜前后角膜内皮细胞形态及角膜厚度各项参数的变化均无统计学意义。结论RGPCL戴镜患者在严格的医疗监控下依从性良好,并发症的发生率较低,未引起明显角结膜的异常。Objective To evaluate compliance and complication of patients wearing RGPCL. Methods Follow- ing - up was performed on patients with RGPCL in Peking University Optometry ＆ Ophthalmology Center. A total of 183 subjects were involved in this study. Aftercare frequency（ 1 week, 1 month,3mon.th,6month,one year）, replacement frequency of RGPCL, as well as the incidence of complication were analyzed. Special examinations of optometry（ A-scan examination, Endothelial cell examination ）at 6month, computer-aid corneal topography examination （3mouth and 6mouth ） were performed. The change of average cell density ,cell size , CV, 6A% and corneal thickness of pre-and ly ,2y,more than 2y were observed. Results The aftercare frequency （ 1 week, 1 month, 3 months, six months, one year ） of Beijing pa- tients was 80.2% ,72.5% ,57.1% ,61.5% ,57.1% respectively,that of the other area patients was 35.4% ,43.1% ,54. 5% ,57.0% ,54.9%, The wearers have a aftercare every 2 -3mouths and lens were replaced every 12 - 15 months. The incidences of corneal staining： there were 22 cases （ 12.1% ）of Beijing patients while 7 eases （3.8%）of other regions; For Beijing patients, the rate of A-scan examination was 83.5% , endothelial cell examination 86.8% at 6month; the rate of computer-aid corneal topography examination was 91.2% at 3 month,36.3% at 6 mouth, For the other regions patients , the rate of A-scan examination was 89.9%, endothelial cell examination was 92.4% at 6month; the rate of computer-aid corneal topography examination was 96.2%% at 3 mouth,41.8% at 6 month; There were no statically significant difference between pre-and post wear for corneal endothelial and corneal thickness. Conclusion All patients with RGPCL have a good compliance under the strict medical control. The incidence of complications is low and without serious cornea complications.
