摘 要:目的判断免疫分型在形态学分类困难的白血病中的鉴别作用。方法应用一组敏感及特异的单克隆抗体,采用间接免疫荧光法测定白血病细胞表面标志。结果检测的白细胞表面均有明显的细胞系列特异性标志表达,其中急性髓细胞白血病(髓系组细胞)2例,其细胞分别表达CD13、CD33、和CD33、;前B-ALL1例(前B细胞),表达CD19、HLA-DR;B-CLL1例,表达CD9、CD10、CD19、CD20、Kappa,Ig轻链呈克隆性改变(Kappa+,Lamda-);3例杂合性急性白血病分别表达CD7、CD9、CD10、CD19、CD13、CD33;CD10、CD19、CD9、CD20、CD13、CD14、CD15、CD33、CD41b和CD7、CD9、CD10、CD19、CD13、CD15、CD33。结论免疫分型可提高白血病的诊断分型准确性,特殊类型的白血病的诊断必须依赖免疫分型。objective To determine the differential diagnostic significance of immunologic classification in the leukemias with undecided morphology. Methods Using a series of the sensitive and extraordinary monoclonal antibodies to check up the cells surface of leukemias with indirect immuno - fluorescence analysis. Results All the cases of leukemias expressed extraordinary markers. 2 cases leukemias were acute myeloid leukemia, the leukemia cell markers separately expressed CD13 ,CD33 ,and CD33; 1 case was pre - B cell acute leukemia with the expression of CD19 ,HLA - DR; 1 case was B cell chronic lymphoid leukemia with the expression of CD9 , CD10, C D19, CD20, Kappa, but Lamda was negative ;3 cases were hybrid acute leukemia, one of 3 cases expressed CD7 ,CD9 ,CD10 ,CD19 .CD13 .CD33, another case expressed CD10 ,CD19 ,CD9 .CD20 .CD13 .CD14 .CD15 .CD33 ,CD41b, third case expressed CD7 ,CD9 ,CD10 ,CD19, CD13 ,CD15, CD33. Conclusison Immunologic classification can improve the accuracy in the diagnosis of acute leukemia. The diagnosis of some special acute leukemia must rely on immunologic classification.
