不同术式治疗重度先天性上睑下垂的疗效比较

摘　　要：目的:探讨额肌瓣悬吊术、异体巩膜额肌悬吊术和阔筋膜额肌悬吊术治疗重度先天性上睑下垂的疗效及优缺点。方法:对重度先天性上睑下垂76例90眼进行手术治疗,其中额肌瓣悬吊术24例30眼,异体巩膜额肌悬吊28例32眼,阔筋膜额肌悬吊24例28眼,分析术后上睑下垂矫正效果,上睑弧度、双重睑形成及并发症发生情况。结果:额肌瓣悬吊术取材方便,复发率低,上睑弧度匀称,但双重睑形成稍差。异体巩膜与阔筋膜额肌悬吊术,双重睑形成良好,但复发率稍高。结论:额肌瓣悬吊术、异体巩膜额肌悬吊术和阔筋膜额肌悬吊术均能有效治疗重度先天性上睑下垂,可根据患者的条件和意愿加以选择术式。Objective：To review and investigate the superiority and disadvantage of three operations for severe congenital blepharoptosis. Methods：76 severe congenital blepharoptosis patients （90eyes） undergoing therapy in our hospital were divided in three groups randomly. 24 patients（ 30eyes ）underwent frontalis muscle flap suspension operation, 28 patients（32eyes ）, frontaliss suspension operation with homologous sclera implanted ,and the other 24 patients（28eyes）,the same operation with fascia lata implanted. Results：The patients who got the frontalis muscle flap suspension operation achieved more persistent effect. The frontaliss suspension operation with homologous sclera or fascia lata implanted offered better fold of upper eyelid. Conclusion：All of the there operations can correct severe congenital blepharoptosis efficiently. Oculists should choose the available therapy according to the requirement and condition of patients.
