胸部肿瘤术后心律失常诊治分析

摘　　要：目的探讨胸部肿瘤患者术后心律失常的发生原因及预防、治疗措施。方法对551例胸部肿瘤患者的临床资料进行回顾性分析。结果术后共发生心律失常86例。〈60岁者术后心律失常的发生率为10．4％，≥60岁者为20．6％；术前合并心血管疾病者术后心律失常发生率为56．4％；肺癌患者术后心律失常发生率为12．5％，其中肺癌患者全肺切除术后心律失常发生率为20．5％，肺叶切除术后心律失常发生率为10．6％，肺楔形切除术后心律失常发生率为8．5％；纵隔肿瘤患者术后心律失常发生率为16．6％；食管癌患者术后心律失常发生率为23．5％，其中颈部吻合者术后心律失常发生率为22．0％，弓上及弓下吻合者为15．3％；贲门癌为20．1％。结论年龄、术前合并心血管疾病、手术方式及术中状况与术后心律失常的发生密切相关。充分的术前准备，加强术后监护，纠正缺氧可以预防或减少心律失常的发生。Objective To discuss the causes of postoperative arrhythmia patients with thoracic tumor and to explore the preventive and therapeutic measures. Methods A retrospective data of 551 patients with thoracic tumor was analyzed. Results There were 86 cases suffering postoperative arrhythmia, among which the incidence was 10.4 % below 60 years old and 20.6 % above. The incidence of those with preoperative cardiovascular diseases was 56.4 % ,of those with mediastinum tumor was 16.6 %. The incidence of those with lung cancer was 12.5 % ,individ- ually 20.5 % for those receiving total pneumonectomy, 10.6 % for pulmonary lobectomy,and 8.5 % for wedge-shape escision of lung. Of those with esophageal tumor the incidence was 23.5 % , individually 22.0 % for those receiving cervical anastomosis,and 15.3 % for intrathoraclc anastomosis. Besides, the incidence of those with cardiac cancer was 20.1%. Conclusions＇ Postoperative arrhythmia on patients with thoracic tumor is intimately correlated with the age, concurrent preoperative cardiovascular diseases,surgical methods and intraoperative conditions. It＇s feasible to prevent and reduce the incidence of postoperative arrhythmia by sufficient preoperative preparation,intensive post-operative custody and corrective anoxia.
