“棉絮征”或“假脂瘤征”对乳腺癌诊断价值的研究

摘　　要：目的:探讨乳腺癌"假脂瘤征"或"棉絮征"(下称:"棉絮征"),并研究其诊断价值。方法:对157例出现"棉絮征"的乳腺癌病人进行回顾性分析,并同"酒窝征"、"橘皮征"进行比较。结果:"棉絮征"的出现频率为7.1%,其乳腺癌的诊断符合率为100%;在乳腺癌各TNM分期中,以属Ⅰ～Ⅱ期居多,其病理学基础是癌细胞索浸润脂肪组织,影像学表现为肿瘤毛刺侵入脂肪组织。结论:同"酒窝征"、"橘皮征"一样,"棉絮征"也可以视为乳腺癌的又一典型临床体征。该体征对乳腺癌的诊断符合率高,有较高诊断价值。Objective To investigate the role of ＂pseudolipoma＂ or ＂fluttering cotton＂ sign in the diagnosis of breast cancer. Methods The data from 157 cases of breast cancer showing the ＂pseudolipoma＂ or ＂fluttering cotton＂ sign, were analized retrospectively. Results The frequency of the designated sign was 7.06%, its diagnostic accuracy of breast cancer was 100%. Most of the patients with this sign were in TNM Stage Ⅰ or Ⅱ. Its pathological mechanism was tumor invasion into the adipose layer, which could be demonstrated radiologically. Conclusions Like the ＂dimple sign＂ and the ＂orange-peel sign＂, the ＂fluttering cotton sign＂ might be considered as another typical clinical sign of breast cancer.
