成人胆总管囊肿的外科治疗(附17例报告)

摘　　要：目的探讨成人先天性胆总管囊肿的特征与外科治疗。方法对我科自1995年4月～2006年8月收治的17例成人先天性胆总管囊肿患者的资料进行回顾性分析。术前B超诊断正确率为94，1％（16／17）；ERCP、MRCP确诊率为100％（11／11）。17例手术病人中：行囊肿切除、肝总管空肠Rounx—Y吻合术6例。结果共随访14例，随访率为82，4％，平均随访57个月，未发现恶变病例。结论B超可作为先天性胆总管囊肿的首选检查。囊肿切除或囊肿部分切除、肝总管空肠Rounx—Y吻合术是成人先天性胆总管囊肿的首选术式。早期根治性手术，是成人先天性胆总管囊肿的首选术式。早期根治性手术，是成人先天性胆总管囊肿诊治的关键。Objective To analyze the morbidity charcterstic and sugical procedure of adult congenital choledochal cyst. Methods Seventeen cases of adult congenital bile cyst treated in our deartment from April 1995 to August 2006. The diagnosed by B- ultrasound accuracy was 94. 1% （16/17） ;ERCP.MRCP accuracy was 100% （11/11 ）. Of all the 17 patients operation with the resection of cyst and hepaticojejunostomy was done in 11 cases ; the resection of part cyst and hepaticojejunostomy was done in 6 cases. Re- suits Fourteen cases were followed up for an average period of 57 months and no malignant changes was found. Conclusions B-ul- trasound should be the first choice of examination while resection of cyst or resection of part cyst and hepaticojejunostomy be the first choice operation. Early radical operation of cyst treatment played key roles in the adult congenital bile duet cyst.
