综合医院内科门诊不同性别患者的抑郁障碍患病率调查

摘　　要：目的了解沈阳市综合医院内科门诊不同性别患者抑郁障碍的患病率。方法采用分层随机整群抽样方法抽取沈阳市不同级别的综合医院23家进行调查,共调查5312例年龄≥15岁的内科门诊患者。调查员在患者就医内科医生前使用扩展的一般健康问卷(GHQ-12)作为筛查工具,将调查对象分为精神疾病高、中、低危险组,然后由精神科医生使用DSM-Ⅳ定式临床检查(SCID-I/P)对高、中、低危险组100%、40%、10%的调查对象进行SCID诊断。结果沈阳市综合医院内科门诊女性患者抑郁障碍的患病率为12·09%〔95%CI为(10·82,13·52)〕,男性患者为7·98%〔95%CI为(6·70,9·52)〕,女性显著高于男性患者,RR值为1·52〔95%CI为(1·24,1·85)〕。结论沈阳市综合医院内科门诊女性抑郁障碍患病率高于男性,应该更加关注综合医院女性患者抑郁障碍的诊治问题。Objective To describe the gender differences in prevalence of depression at internal medicine outpatient departments of general hospitals in Shenyang. Methods Investigation was performed in 23 general hospitals of Shenyang by stratified random cluster sampling. A total of 5312 outpatients above the age of 15 were enrolled. Using expanded version of General Health Questionnaire （GHQ -12） as a screen instrument, we divided the samples into a high -risk group, a moderate -risk group and a low -risk group. SCID diagnosis was done on the investigated subjects （ 100% of the high - risk group, 40% of the moderate - risk group and 10% of the low - risk group） by structured clinical interview of DSM - Ⅳ Axis I Disorders - Patient Edition （ SCID - I/P）. Results The morbidity rate of depressive disorder in the females was 12. 09% [95%CI （10. 82, 13.52） ], and in the males was 7. 98% [95%CI （6.70, 9. 52） ] ; the value of RR was 1.52 [95% CI （ 1.24, 1.85） ]. Conclusion The prevalence of depressive disorder is higher in females than in males at internal medicine outpatient departments of general hospitals. Closer attention should be paid to the diagnosis and treatment of depressive disorder in female patients.
