摘 要:目的探讨肝血管瘤周围灌注异常(HPD)现象的多层螺旋CT特点。资料与方法回顾性分析220例患者共386个肝血管瘤的多层螺旋CT表现,观察肝灌注异常形态、位置,分析其与血管瘤大小及强化速度的关系。结果386个血管瘤中出现灌注异常114个(29.5%)。快速强化型血管瘤出现HPD现象的发生率(48.8%)远远大于慢速强化型(8.2%)(P<0.001);直径≤2cm小血管瘤HPD的发生率(35.3%)高于直径>2cm大血管瘤(18%)(P<0.005)。结论灌注异常现象在肝血管瘤中并不少见,更常见于快速强化型小血管瘤。Objective To evaluate the multislice CT (MSCT) findings of the hepatic perfusion disorder (HPD) around hepatic cavernous hemangiomas (HCH). Materials and Methods Multiphasic dynamic MSCT findings in 220 patients with 386 hemangiomas were reviewed retrospectively. The relationship between HPD around HCH and the size of HCH and enhancement pattern was assessed. Results HPD was detected in 114 of the 386 hemangiomas (29.5%). The prevalence of HPD was significantly higher in hemangiomas with rapid enhancement (48.8%) than that in hemangiomas with slow enhancement (8.2%) (P 〈0. 001 ). Small HCH (less than 2 cm in the diameter) showed more HPD than large HCH (more than 2 cm in the diameter) (P 〈 0. 005). Conclusion HPD was not uncommon in HCH, which was more common in small HCH with rapid enhancement.
