老年性白内障人工晶状体眼的调节融合和立体视研究

摘　　要：目的应用综合验光仪和同视机立体视图检查白内障患者术后的调节、融合和立体视结果,观察其双眼视觉功能恢复情况。方法收集单眼白内障患者36例36眼,双眼白内障患者32例64眼,观察其调节、融合和立体视恢复情况,同期收集30名同龄健康人作为对照。结果通过统计分析发现3组人群的调节因素中,调节近点、正负相对调节、调节反应和调节幅度差异均有统计学意义(P<0.05)。在调节与融合因素中,3组人群的调节性集合与调节比值(AC/A)差异无统计学意义(P>0.05);3组人群的融和范围比较,差异有统计学意义(P<0.05)。在立体视因素中,3组人群的Titmus和颜氏随机点立体视差异有统计学意义(P<0.05),同视机检查远立体视差异无统计学意义(P>0.05)。结论白内障术后无论单眼或双眼仍然存留一定的调节能力,且具有一定的融合与立体视功能。双眼白内障组其调节融合与立体视优于单眼白内障组,但与对照组相比仍有差异。Objective To observe the visual accommodation, fusion and stereopsis at cataract patients with comprehensive optometry and synoptophore postoperatively, and to observe the recovery of binocular visual function. Methods Thirty-six cases （36 eyes） with unilateral cataract patients,32 cases（64 eyes） with binocular cataract patients and 30 health people as controls were selected for this study. The recovery of visual accommodation, fusion and stereopsis were observed. Results Among the three groups, there was significant difference in near point of visual accommodation,positive related accommodation,negative related accommodation, accommodation response and amplitude of accommodation （ P 〈 0.05 ）. There was not significant difference in accommodative convergence and AC/A among the three groups （ P 〉 0. 05 ）. There was significant difference in fusion among the three groups （ P 〈 0. 05 ）. There was significant difference in the Titmus and RDS stereopsis among the three groups（P 〈0. 05）. There was not significantly difference in the synoptophore stereopsis among the three groups （P〉0. 05）. Conclusion The postoperative eyes（bilateral cataract or unilateral cataract） retain accommodation,fusion and stereoscopic vision function partly. The accommodation,fusion and stereoscopic fimction of the postoperative patients in bilateral cataract group were superior to unilateral cataract group, but both were inferior to the control grouup.
