PAX-5在小细胞肺癌和淋巴瘤诊断中的意义

摘　　要：目的探讨B细胞特异性激活蛋白(PAX-5)在淋巴瘤及小细胞肺癌诊断中的应用价值。方法分析应用免疫组化技术得出明确诊断、且进行了PAX-5染色的70例淋巴瘤(34例非霍奇金B细胞淋巴瘤、36例霍奇金淋巴瘤)及33例肺小细胞癌的病理结果,总结其在实际诊断工作中的意义。结果PAX-5在非霍奇金B细胞淋巴瘤瘤细胞核中均呈弥漫强(+),且阳性率及阳性强度均高于CD79a;在经典型霍奇金淋巴瘤中,PAX-5阳性细胞的大小与CD30阳性细胞对应,并且其阳性强度明显弱于周围B淋巴细胞,此为重要的判断依据。33例肺小细胞癌中,22例PAX-5(+),其阳性细胞的百分率和阳性强度并不优于Syn和CD56染色,但确有Syn和CD56均(-)仅PAX-5阳性的小细胞癌。结论PAX-5是CD20、CD79a阴性B细胞淋巴瘤的重要诊断依据,其阳性表达率高于CD79a;PAX-5在霍奇金淋巴瘤的诊断和鉴别诊断中具有重要意义,在肺小细胞癌诊断中可作为Syn和CD56阴性时的补充。Objective To investigate the significance of B cell-specific activator protein （PAX-5） in diagnosis of lymphoma and small cell carcinoma of the lung. Methods PAX-5 immunohlstochemistry was applied in 70 cases of lympbomas （34 nonHodgkin B cell lymphomas and 36 Hodgkin lymphomas） and 33 cases of small cell carcinomas of the lung during routine work. Results The expression of PAX-5 showed diffuse and strong nuclear positivity in non-Hodgkin B cell lymphomas. The percentage and intensity of the positive cells were more than those of CD79a. In Hodgkin lymphomas, the PAX-5-positive cell was large, comparable with CD30-positive cell, but was weaker than surrounding B lymphocytes. Among small cell carcinomas of the lung, 22 cases were positive for PAX-5. The percentage and intensity of the positive cells were not more than those of Syn and CD56. Conclusion PAX-5 may play an important role in the diagnosis of B cell lymphomas when CD20 or CD79a is negative, and the positive percentage is higher than that of CD79a. It is essential in differential diagnosis of Hodgkin lymphomas, and can also be taken as a complementary antibody to apply when Syn or CD56 are both negative in the diagnosis of small cell carcinoma of the lung.
