大肠癌细胞增殖的病理意义与预后相关性研究

摘　　要：目的:研究大肠癌细胞增殖的病理意义与预后相关性。方法:大肠癌手术切除标本88例,均作常规病理检查及检测增殖细胞核抗原(PCNA)表达,观察在不同临床病理因素中癌细胞PCNA表达的差异与意义,并作预后相关性的分析。结果:PCNA可表达于正常肠粘膜基底层细胞核中呈极性分布,在癌细胞中广泛表达,并呈异质性分布,其表达强度显著高于正常肠粘膜(P<0.05)。大肠癌中PCNA表达与肿瘤细胞分化、Duck分期及淋巴结转移情况均有显著相关(P<0.05)。癌中PCNA强阳性表达的患者其5年生存期显著低于弱表达组(P<0.05)。结论:在大肠癌常规病理诊断中联合PCNA检测,对疾病的进展与预后评估有一定的价值。Object： To study the correlation between the pathological significance and prognosis of the proliferation of colorectal carcinoma. Methods： In order to investigate the difference of the PCNA expression in cancer cells with different clinical pathological factors and its significance, 88 cut off specimen from colon surgical operation were used for routine pathological examination and immunohistochemical detection of PCNA, and the analysis of prognosis dependability was also performed. Results： PCNA was expressed in normal cells at basal layer of the intestinal mucosa and it was in polar contribution. While in cancer cellsion, it had a widespread expression pattern with a heteroid distribution. The expression intensity in cancer cells was significantly higher than that in normal cells. The expression of PCNA in colon cancer cells have predominant correlation with cell differentiation, Duck staging and lymphatic metastasis（P〈0.05）. The five year survival rate of the patients with strong expression of PCNA was much lower than that of the weak expression groups （P〈0.05）.Conclusion： In pathological diagnosis of the colon cancer, the combining use of the immunohistochemical investigation of PCNA may have some value in progression of the disease and evaluation of the prognosis.
