^(67)Ga显像在结外淋巴瘤诊治中的应用

摘　　要：目的探讨67Ga显像在结外淋巴瘤诊治中的应用价值。方法37例经病理证实为非霍奇金淋巴瘤(NHL)的结外淋巴瘤患者,其中14例患者治疗前行67Ga显像;23例患者均经过6~8个疗程化疗后进行67Ga显像,其中11例伴有手术、2例伴有局部放疗。23例治疗后患者的平均随访时间为26个月(12~48个月)。结果在结外淋巴瘤的诊断中,67Ga显像的灵敏度为71%(10/14),其中10例原发于横膈以上的结外淋巴瘤患者中有6例阳性,4例原发于横膈以下的结外淋巴瘤患者全部阳性,67Ga显像的灵敏度与结外淋巴瘤的恶性程度相关。在结外淋巴瘤治疗后的监测中,67Ga显像的阴性预测值为72%(13/18)。结论67Ga显像不但有助于结外淋巴瘤的诊断,在评价结外淋巴瘤治疗的预后方面同样也有价值。Objective To assess the usefulness of ^67Ga scintigraphy in the diagnosis and in the evaluation of therapy of extranodal malignant lymphoma（EML） patients. Methods Thirty-seven patients with extranodal non-Hodgkin＇s lymphoma（NHL）proved by pathology were studied.Fourteen patients were studied before the treatment,and 23 patients were studied after 6～8 cycles of chemotherapy（including 11 treated with surgery and 2 with limited radiotherapy）.All the 23 patients were followed up in an average time of 26 months（12～48）. Results ^67Ga scintigraphy sensitivity was 72%（10/14） in the diagnosis of EML.^67Ga scintigraphy found abnormalities in 6 of 10 patients whose extranodal lymphoma occurred in supra-diaphragram and all of 4 patients whose extranodal lymphoma occurred in infra-diaphragram.The sensitivity of ^67Ga was correlated with the grade of lymphoma.During the period of follow-up,the negative predictive value（NPV） was 72%（13/18）. Conclusion ^67Ga scintigraphy is helpful to the diagnosis and to evaluate the therapeutic effect in the patients with EML.
