近端股骨转移性肿瘤的手术治疗

摘　　要：目的探讨近端股骨转移性肿瘤的手术治疗指征和疗效。方法对13例股骨近端转移性肿瘤患者行手术治疗,分别采用人工假体置换、动力髋螺钉（DHS）、Gamma钉以及股骨近端髓内钉（PFN）固定治疗,对患者术后疼痛情况、患侧肢体功能、并发症和生存时间等进行随访。结果所有患者疼痛均得到缓解,11例术后可下床行走,2例术后卧床。所有患者术后未出现伤口感染、不愈合、肺炎和褥疮等相关并发症。患者术后生存时间2～24个月,平均8.7个月。结论采用假体置换、DHS、Gamma钉或PFN手术内固定,可有效治疗近端股骨转移性肿瘤,部分可恢复行走功能,并能有效缓解疼痛,有助于提高患者生存质量。Objective To explore the clinical outcomes and indications of surgical intervention in metastatic tumors in the proximal femur region. Methods Thirteen patients with metastatic tumors in the proximal femur region were treated with surgical procedures.Prosthetic replacement and dynamic hip screw（DHS）,Gamma nail or proximal femoral nail（PFN） internal fixation were performed.All the patients were followed up for postoperative pain,limb motor function,complications and survival time. Results All the patients experienced relief of pain postoperatively.Eleven patients could walk after operation,while the other 2 could not.There were no such postoperative complications as wound infection,disunion,pneumonia and bedsore.The average survival time was 8.7 months（2-24 months）. Conclusion Prosthetic replacement,DHS,Gamma nail and PFN nail internal fixation can be used safely in patients with metastatic tumors in the proximal femur region for pain relief and recovery of ambulatory function.It can also improve the quality of life of the patients.
