摘 要:目的探讨环氧化酶-2(COX-2)对甲状腺乳头状癌淋巴管生成的影响及与淋巴结转移的关系。方法运用Envision免疫组化法测定60例甲状腺乳头状癌患者和15例癌旁正常甲状腺组织病理标本COX-2、血管内皮生长因子C(VEGF-C)的蛋白表达,并进行评分,同时用D2-40显色淋巴管进行淋巴管密度(LVD)计数。结果甲状腺乳头状癌中COX-2、VEGF-C较正常组织表达明显增高;有淋巴结转移的甲状腺乳头状癌患者中COX-2、VEGF-C的阳性率明显高于无转移组(P<0.05;P<0.05);转移组LVD计数显著高于无转移组(P<0.01);COX-2与LVD(r=0.71;P<0.01)、VEGF-C与LVD(r=0.68;P<0.01)、COX-2与VEGF-C之间(r=0.65,P<0.01)存在显著正相关。结论COX-2对甲状腺乳头状癌患者的淋巴管形成起着重要的作用,可能通过上调VEGF-C的表达促进肿瘤淋巴管的形成,有利于甲状腺乳头状癌淋巴结的转移。Objective To study expression of cyelooxygenase-2 ( COX-2 ) and its relationship with lymphangiogenesis and lymph node metastasis in papillary thyroid carcinoma. Methods The expressions of COX-2 and VEGF-C were immunohistochemically detected by use of envision method in 60 papillary thyroid carcinoma cases and 14 normal tissue cases nearby tumors respectively, meanwhile the number of lymph vessel density with D2-40 staining positive was obtained. Results The expressions of COX- 2 and VEGF-C were higher than thai of normal tissues. The positive tales of COX-2 and VEGF-C with lymph node metastasis in papillary thyroid carcinoma were siginificantly higher than that without lymph node metastasis (P 〈0. 05;P 〈0. 05). The number of LVD with D2-40 staining in papillary thyroid carcinoma with lymph node metastasis was remarkablely higher than that without lymph node metastasis ( P 〈 0. 01 ). There were siginifcantly positive relationship between COX-2 and LVD in papillary thyroid carcinoma( r = 0. 71 ,P 〈 0. 01 ), as well as siginifcantly positive relationship between VEGF-C and LVD( r = 0. 68,P 〈0. 01 ) . In the meantime the expression of COX-2 was closely related with the expression of VEGF-C in papillary thyroid carcinoma( r = 0. 65,P 〈 0. 01 ). Conclusion COX-2 may play an important rate in the lymphangiogenesis by up-regulalion expression of VEGF-C and promote lymph node metastasis in papillary thyroid carcinoma.
