透明角膜切口位置对人工晶状体眼高阶像差的影响

摘  要:目的探讨不同透明角膜切口位置对白内障术后人工晶状体眼高阶像差的影响。设计回顾性病例系列研究对象老年性白内障患者46例(60眼)。方法将患者分为两组,角膜逆规散光或无散光行颞侧透明角膜切口者为A组(30眼);角膜顺规散光行上方透明角膜切口者为B组(30眼)。行超声乳化白内障吸除联合人工晶状体植入术,术后3个月使用Zywave波前像差分析仪,在瞳孔直径6.0 mm时,测量人工晶状体眼的各项波前像差指标,并进行比较。主要指标Z(3,-3)到Z(4,-4)的Zernike常数、三阶像差、四阶像差和整体商阶像差。结果A组和B组的Zemike常数中Z(4,2)的值分别为(0.10±0.07)μm和(0.05±0.05)μm,差异有统计学意义(P=0.01),其余的Zernike常数、三阶像差、四阶像差和整体高阶像差各指标间两组比较差异均无统计学意义(P均>0.05)。结论白内障手术中根据角膜散光情况确定不同角膜切口的位置对术后人工晶状体眼高阶像差影响较小。(眼科,2008,17:86-88)Objective In order to obtain the objective evaluation of the optical quality of the pseudophakic eyes, the influence of different clear-corneal incisions on the hlgh-order wavefront aberrations were explored. Design Retrospective case series. Participants 46 cataract patients (60 eyes). Methods The eyes were divided into two groups aecording to the astigmatism of the cornea. Group A (30 eyes) were eyes with astigmatism against rule or without astigmatism and operated at the temporal-side clear corneal incision, while Group B (30 eyes) were eyes with astigmatism with rule and operated at the superior-side clear corneal incision. Three months after cataract surgery, aberrations in the eyes were measured in dilated pupil (6 mm) using a Hartmann-Shack-type aberrometer (Zywave) and compared in the two groups, also compared in each group. Main Outcome Measures Individual Zemike polynomials and the root-mean-square (RMS) of 3rd- and 4th-order aberrations, total high order aberration. Results The Z (4,2) of Zernike coefficient in group A and group B was (0.10±0.07) μm and (0.05±0.05) μm respectively. The difference was significant (P=0.01). The other Zernike coefficients did not differ significantly between two groups (P〉0.05), nor did the RMS value of the 3rd- and 4th-order aberrations and the total high order aberration. Conclusion No significant differences in high-order aberrations among eyes are demonstrated after surgery with different clear-corneal incisions.
